ObjectiveTo evaluate the role of combined high resolution ultrasonography and 99mTc-MIBI scintigraphy in the diagnosis of secondary hyperthyroidism?SHPT?and localization of parathyroid adenoma.Materials and MethodsClinical data of 134 patients with SHPT,who underwent operation in the second affiliated hospital of Anhui Medical University between Jan.2011 and Dec.2016 were retrospectively analyzed.It was analyzed that the sonographic characteristics in this study,included the reasons of misdiagnosis and missed diagnosis.These two imaging modalities were also compared.ResultsThere were 513 parathyroid lesions in 134 SHPT patients proved by pathology,including9 ectopic lesions.The sensitivity,specificity and PPV for preoperative imaging were65.35%,62.45%and 97.06%for ultrasonography,and 70.59%,65.63%and 96.64%for99mTc-MIBI scintigraphy.There were no significant difference between ultrasonography and 99mTc-MIBI scintigraphy??2=0.919,P=0.338;?2=0.187,P=0.665;?2=0.098,P=0.755?.The accuracy preoperative localization of ultrasonography and 99mTc-MIBI were42.11%?40/95??81.05%?77/95?,the accuracy of 99mTc-MIBI was obviously higher than that of ultrasonography??2=22.79,P<0.05?.Conclusions99mTc-MIBI scintigraphy is complementary to ultrasonography.Both of ultrasonography and 99mTc-MIBI scintigraphy should be used before operation routinely to localize parathyroid lesions.
