| Objective 1 Quantitative study: cross-sectional survey(1)To understand the psychological distress of patients with advanced lung cancer in rural areas during chemotherapy,and to understand the specific problems causing psychological pain of patients with lung cancer undergoing chemotherapy through the list of questions in rural areas;(2)to analyze the current situation of coping methods of psychological distress of rural lung cancer chemotherapy patients;(3)to explore the influence factors of psychological distress in patients with advanced lung cancer undergoing chemotherapy in rural areas.2 Qualitative research:By using the phenomenological method of qualitative research and semi-structured interview,the subjective feeling and coping experience of psychological distress in rural patients with advanced lung cancer undergoing chemotherapy were deeply explored.Methods This study consists of two parts.In the first part,a quantitative study was conducted to select 107 rural patients with advanced lung cancer who were hospitalized for chemotherapy in a third-grade first-class hospital in Anhui province through cross-sectional investigation.General situation questionnaire,psychological distress screening tool(DM)and cancer coping style scale(CCMQ)were used for investigation,and SPSS 22.0 was used for statistical analysis.The second part,qualitative research,through phenomenological research methods,according to the research purpose,9 cases selected psychological distress DT a score of 4 or more points are semi-structured depth interview in order to get access to patients about psychological distress and coping styles of true feelings and inner experience,Claizzi phenomenological data analysis of interview data were analyzed.Results 1 Quantitative study:(1)107 cases of rural patients with advanced lung cancer chemotherapy in 60.75% of patients were found to exist in the clinical significance of psychological distress,the top 10 of the order of psychological distress and specific questions is: economic pressures(70.09%),lack of disease inspection related information(69.16%),bringing the burden to the family(63.55%),could not take care of their family(60.75%),sleep disorder(45.79%),cannot continue role before disease(44.86%),fear(39.25%),difficulty getting along with children(34.58%),anxiety(31.78%),anxiety(28.04%);(2)Five dimensions of coping style: the scores were 14.59 + 3.95,10.51 + 3.65,10.44 + 3.59,8.22 + 2.77,and 7.22 + 2.91;(3)According to the regression analysis results,offspring,family per capital monthly income,pain,yielding coping style and fantasy coping style were finally entered into the regression equation,which accounted for 41.7% of the total variation of psychological distress.2 Qualitative research: Five psychological distress subjective feelings and eight coping experiences were extracted.(1)The subjective feelings of psychological distress associated with disease: helpless;self-accusation caused by the role behavior conflicts;inappropriate attribution way;the sense of uncertainty towards disease;unbearable pressure.(2)The coping experience of psychological distress: positive self-caring;desire and seeking support;actively seeking disease related knowledge and information;avoiding socializing subjectively;acceptance and submission;constrained and compromised;positive self-motivation,reducing symptoms and restore confidence.Conclusion1The psychological problems of patients with advanced lung cancer undergoing chemotherapy in rural areas should not be ignored.In this study,60.75%(65/107)of patients had a psychological distress(DT)scores greater than or equal to 4,which constituted the specific psychological problems of patients with advanced lung cancer on chemotherapy in rural areas,including family,practical,emotional and physical problems.2 Patients tend to select coping styles of confrontation;3Offsping,per capital monthly income of households,having pain or not,yielding coping and fantasy coping are the main influencing factors of psychological distress.4Psychological distress subjective experience of the patients are complex and changeable,and their coping experiences is relatively rich.5Qualitative research results provide an important supplement and interpretation to the descriptive research results,and find that patients' cognition of disease,experience of distress and coping are deeply influenced by traditional Chinese culture.Based on the psychological distress of patients,the medical staff directed the patients to adopt appropriate coping methods,providing reference basis for the psychological distress intervention of patients with lung cancer chemotherapy in the future. |
