Objective The purpose of this study was to investigate the depressive symptoms,somatic symptoms,sleep quality,course of disease,and severity of sexual dysfunction in the first-episode depressive patients without medication,and to explore the factors affecting the sexual life quality in the first-episode depressive patients.Methods The Somatic Self-rating Scale(SSS),Montgomery and Asberg Depression Rating Scale(MADRS),Pittsburgh Sleep Quality Index(PSQI),and Arizona Sexual Experience Scale(ASEX)were used in this paper.The study compared and evaluated118 first untreated depressive patients(Study Group)and 106 healthy controls matched(Control Group)by age,sex,and education.Results 1.The total scores of SSS,MADRS and PSQI in the study group were significantly higher than those in the control group(P < 0.05 or P < 0.001).The total score of ASEX and the scores of each factor in the study group were significantly higher than those in the control group(P < 0.05 or P < 0.001).2.The total score of SSS and each factor score(except blurred vision)were positively correlated with the total score of ASEX and each factor(P<0.001).There were no significant correlation between blurred vision and sexual drive(P > 0.05).The top three somatization factors related to sexual drive were headache and dizziness(r = 0.780,P <0.001),sweating,cold and heat(r = 0.742,P < 0.001),cardiovascular symptoms(r =0.732,P < 0.001);the top three somatization factors related to sexual arousal were urinary tract symptoms(r = 0.790,P< 0.001),headache and dizziness(r = 0.786,P <0.001),cardiovascular symptoms(r = 0.738,P < 0.001).The top three somatization factors related to vagina lubrication/ erection of penis were headache and dizziness(r =0.815,P < 0.001),urinary tract symptoms(r = 0.707,P< 0.001)sweating,cold and heat(r = 0.688,P < 0.001);the top three somatization factors related to orgasm capacity were headache and dizziness(r = 0.701,P < 0.001),cardiovascular symptoms(r =0.680,P < 0.001),sweating,cold and heat(r = 0.568,P < 0.001);the top three somatization factors related to sexual satisfaction were cardiovascular symptoms(r =0.769,P < 0.001),headache and dizziness(r = 0.764,P < 0.001),and urinary tract symptoms(r = 0.732,P < 0.001).The total score of MADRS was positively correlated with the total score and all factors of ASEX(P < 0.001).Among the ASEX factors,the most close relation to the total score of MADRS was sexual arousal(r =0.866,P <0.001).The total score of PSQI was positively correlated with the total score and all factors of ASEX(P < 0.001).Among the ASEX factors,the most relevant part of the total score of PSQI was vagina lubrication / erection of penis(r =0.781,P < 0.001).Duration of disease was positively correlated with total score of ASEX and all factors(P < 0.001).Among the ASEX factors,sexual satisfaction was most related to the duration of disease(r =0.749,P < 0.001).3.Took ASEX total score as dependent variable,SSS factor score,MADRS score,PSQI score,and the duration of disease as independent variables in multivariate regression analysis,which showed that SSS factor score(except blurred vision),MADRS score entered the regression equation.Conclusion Based on the above findings,we can draw the following conclusions:1.Patients with depression have prminent sexual dysfunction and somatization symptoms.2.Depression degree,somatization symptoms,sleep quality,and the duration of disease are important factors affecting the quality of sexual life.3.In depressive patients with somatization symptoms,the somatization symptoms of headache/dizziness,cardiovascular symptoms,urinary tract symptoms, sweating/chills/fever are the main factors affecting sexual function.4.Depressive degree and somatization symptoms(except blurred vision)have a good predictive effect on sexual quality of life,of which cardiovascular symptoms,headache,dizziness,urinary tract symptoms are more predictive of sexual function. |
