Objective:Based on CT lacrimal radiography,systematically measuring and studying related parameters of main physiologic structures of chronic dacryocystitis patients and retrospective analysis of chronic dacryocystitis Chinese medicine literature,providing reference for accurate treatment of chronic dacryocystitis in Chinese and Western medicine.Methods :A total of 178 patients were diagnosed with chronic dacryocystitis by CT lacrimal radiography.Statistical analysis was performed on the patient’s general conditions such as gender,age,and incidence of eye diseases.After CT lacrimal radiography,CT post-processing techniques were used.Nasal lacrimal duct imaging was performed axially,coronally,and sagittally.The images of bone nasolacrimal ducts were selected and used to measure the length of bony nasolacrimal duct on the CT image,and the origin and end of the nasolacrimal duct.The cross-sectional area,the angle between the inferior turbinate and the medial side wall of the maxilla(lower turbinate angle),the angle between the long axis of the lacrimal sac and the bony nasolacrimal duct(relative to the lacrimal sac bone nasolacrimal duct angle)were measured.SPSS21.0 software was used to analyze the relationship between the data and the correlation with the disease,and a retrospective analysis of the Chinese literature of chronic dacryocystitis.Result:1.There are more women than men,with a male to female ratio of 1:4.23.2.The patient’s age ranges from 20 to 84 years,with an average age of 51.21±13.87 years and a median of 50.5 years;the disease is mainly distributed in 40-60 years old,accounting for 49.44%;the age difference of male and female patients and their distribution in all age groups are no significance(P>0.05).3.The monocular morbidity(88.76%)is significantly higher than the binocular morbidity(11.24%).There is no statistically distribution of the male and female patients on the pathological eye(P>0.05).4.There is no statistically difference in the cross-sectional area of the tip and end of the nasolacrimal duct between the affected and non-affected eyes(P>0.05);the cross-sectional area of the initial segment of the ocular nasolacrimal duct in the affected and non-affected eyes is smaller than the terminal cross-sectional area(P<0.05).5.The average length of the nasolacrimal duct is 11.13±2.09 mm,the minimum is 6.20 mm,and the maximum is 17.40mm;There is no statistically difference in the length of ocular nasolacrimal ducts between affected eyes and non-affected eyes(P>0.05).6.The average inferior turbinate angle of the patient is 78.22°±20.97°,the minimum is 31.8°,and the maximum is 142.6°;There is no significant difference in the ocular turbinate angles between the affected eyes and the non-affected eyes(P>0.05).7.The mean relative lacrimal sac BNLD angle of patients is 23.21°±9.87°,the minimum value is 1.1°,and the maximum value is 62.7°.There is no significant difference in the angle of lacrimal sac BNLD between affected and non-affected eyes in all patients and male patients(P>0.05).The angle of lacrimal sac BNLD in affected eyes of female patients is larger than that of non-diseased eyes(P<0.05).8.There are no statistically differences in the cross-sectional area,length,inferior turbinate angle,and lacrimal sac BNLD angle of the affected eyes and nose at different ages(P>0.05).The cross-sectional area of the tip of the nasal canal at the age of 20-40 years is smaller than that of the 41-60 years old and 61-84 years old(P<0.05);The angle of BNLD in 41-60-year-old patients is larger than that in 61-84 years old(P<0.05).9.The cross-sectional area of BNLD at the beginning of the diseased eye is positively correlated with the cross-sectional area of the end(R=0.486,P<0.01);The cross-sectional area of BNLD at the beginning of the diseased eye is negatively correlated with BNLD length(R=-0.212,P<0.01).The cross-sectional area of the initial end of BNLD in the affected eyes is positively correlated with the angle of lacrimal sac BNLD(R=0.140,P<0.05).The cross-sectional area of the BNLD end of the diseased eye is positively correlated with the inferior turbinate angle(R=0.145,P<0.05).There is a positive correlation between the BNLD angle and the inferior turbinate angle in the affected eye(R=0.166,P<0.05).10.There is a positive correlation between the cross-sectional area of the BNLD end of the non-affected eye and the cross-sectional area of the end(R=0.530,P<0.01);the cross-sectional area of the BNLD end of the non-affected eye is negatively correlated with the angle of the lacrimal sac BNLD(R=-0.157,P<0.05).Conclusion:1.Middle-aged and elderly women are the main populations of chronic dacryocystitis.Chinese medicine believes that this may be related to the hepatorenal blood deficiency in middle-aged and elderly women.2.The narrow nasolacrimal duct diameter,narrow BNLD length,narrow inferior turbinate angle,and large relative lacrimal sac BNLD angle may be risk factors for the development of chronic dacryocystitis.
