Objective To evaluate the diagnostic significance of bacterial pneumonia in elderly patients using serum procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC),and neutrophil lymphocyte ratio(NLCR)in order to guide clinical management and to assess therapeutic response.To study the clinical characteristics of CURB-65 score and fasting blood glucose(FBG)level in this population.Methods One hundred and sixty-two cases of diagnosed community-acquired pneumonia(CAP)in elderly admitted to respiratory medicine service at Tianjin Port Hospital between January and July 2015 were selected.Patients were divided into bacterial and non-bacterial pneumonia groups according to the sputum culture,smear,and bronchoalveolar lavage collected.Eighty elderly subjects who underwent routine examinations were randomly chosen from our outpatient department as the control group.PCT,CRP,WBC,NLCR,CURB-65 score and FBG were collected for all patients.Results: The diagnostic cut-off points for elderly CAP for PCT,WBC,CRP and NLCR were 1.55ng/ml,12.5 x 109/L,21.5mg/L,and 7.7,respectively.The area under the curve were 0.991,0.660,0.740,0.969,respectively.The PCT,CRP,WBC,NLCR,CURB-65 score and FBG were the higher in bacterial than non-bacterial pneumonia group.They were normal in control group.In subgroup analysis,higher number of diabetic patients with sepsis in bacterial than non-bacterial pneumonia group(p <0.01).Conclusion: In community-acquired pneumonia in elderly patients,PCT is a good diagnostic indicator followed by NLCR,while CRP's diagnostic value is moderate and WBC is low.FBG was significantly elevated in CAP patients,higher in bacterial than non-bacterial groups,and positively associated with severity of infection.
