Aims: To investigate the prevalence of hyperuricemia in middle-aged and young people in Tianjin and to explore the relationship between hyperuricemia and cardiovascular risk factors in young and middle-aged people. Methods: A random sample was used to select adults over the age of 18 and under 65 for medical examination at Tianjin Medical University General Hospital Medical Center from January 2015 to January 2016.The survey included chronic medical history such as history of hypertension, history of hyperlipidemia, history of diabetes, history of cancer, medication taking, smoking history, history of drinking.Patients were examined for height, weight, waist circumference,Systolic blood pressure, diastolic blood pressure, blood lipids, blood glucose, serum creatinine, alanine aminotransferase, blood uric acid,2 hours postprandial glucose,C-reactive protein and high-sensitivity C-reactive protein.Hypertension, diabetes, smoking, and overweight/obesity are the major risk factors for the development of cardiovascular disease(CRFs)806 subjects were divided into cardiovascular risk factor(CRF)group(699 cases)and non-CRF group(107 cases);806 subjects were divided into male group(476 cases).CRF(455 cases)had no CRF(21 cases),female group(330 cases),among which CRF(244 cases)had no CRF(86 cases);806 subjects were divided into male group(476 cases)including(hyperuricemia)HUA(102 cases),(non-hyperuricemia)Non-HUA group(374 cases),female group(330 cases)including HUA(23 cases),Non-HUA(307 cases);806 subjects were divided into three groups according to the CRFs aggregation situation. The first group: CRF ? 1(277 cases);the second group: CRFs = 2(218 cases);the third group: CRFs ? 3(311 cases);806 subjects were divided into HUA(125 cases),Non-HUA(681 cases),and then stratified by age. They were divided into youth group(aged 18 to 40 years old)(119 cases)and middle age group 41 to 65 years old group(687 cases).Except for(1)severe heart failure(NYHA Class IV),(2)renal insufficiency(Scr>265umol / L);(3)malignant tumor;(3)other diuretics or uric acid lowering drugs within 14 days prior to admission or physical examination during uric acid levels were missing;(5)during pregnancy;(5)secondary hypertension in patients.According to whether there is a cardiovascular risk factors, whether there is hyperuricemia group data subjects. The data of the numerical variables were expressed as x±s,group t-test was used for comparison among groups,n(%)was used for categorical variables, and the Chi-squared test was used for comparison between groups,If does not meet the chi-square test,Fisher's exact test is used.Multivariate analysis Logistic multiple regression model was used to analyze the association between hyperuricemia and cardiovascular risk factors in different subgroups.Logistic regression model was used to calculate OR(95% CI)values.Two-tailed P <0.05 was defined as statistically significant.RESULT : 1.A total of 806 people were selected,476 were males and 330 were females. The prevalence of hyperuricemia in people with CRFs was 17.9%,higher than those without CRFs,the difference was statistically significant(P <0.001).Further study found that the prevalence of hyperuricemia in men with CRFs was 22.4% higher than that of women 9.4%,the difference was statistically significant(P <0.001).2.Male hyperuricemia incidence of 21.4%,female hyperuricemia incidence of 7.0%.In the HUA population,BMI,WC,TC,TG,CR,SBP were higher than those in uric acid, and HDL was lower than that in uric acid normal group.3.two and greater than or equal to three CRFs in the normal uric acid group were 27.46% and 32.75%(29.41% and 50.00% for men and 25.08% and 11.73% for females, respectively),There are two and three or more CRFs in young people with hyperuricemia, which are 24.80% and 70.40% respectively(male 20.59%,74.51%,female 43.48%,52.17%).4.The incidences of ?1,?2,?3 and ?4CRFs in hyperuricemia group were significantly higher than those in normal uric acid group(100%,95.2%,70.4%,24.8%,84.3%,60.2%,32.7%,16.0%,There was a significant statistical difference between the two(P <0.001).Conclusion: The incidence of hyperuricemia and CRFs clustering is high, and hyperuricemia in middle-aged and young people in Tianjin is significantly related to the clustering of CRFs.In patients with hyperuricemia,we should pay more attention to the possibility of cardiovascular complications. Therefore, further research to reduce serum uric acid levels provide a new target for cardiovascular disease or possible new treatment,Especially in female patients.
