Effects Of Xiaorong Wenban Fang On Unstable Angina Plaques And TNF-α,hs-CRP And Ampk

Unstable angina pectoris,one of the non-ST-segment elevation acute coronary syndrome,is one of the important causes of the death of cardiovascular disease.It is found that the main pathological basis of unstable angina pectoris is the erosion and rupture of unstable atherosclerotic plaques,and the inflammatory reaction runs through the whole disease.In recent years,Chinese medicine has made good progress in unstable angina pectorist.This topic through the effects of Xiaorong Wenban Fang on unstable angina pectoris and its influence on inflammatory factors,to explore the anti-inflammatory effect of Wenban dissolving Wenban recipe,and to discuss its mechanism of action.This study is divided into two parts:Experiment one:Effect of Xiaorong Wenban Fang on plaque of patients with unstable angina pectoris and the effect of serum TNF-αand hs-CRPObject:To observe the clinical effect of Xiaorong Wenban Fang of unstable angina with Qi deficiency phlegm and blood stasis syndrome patients.And observe the effect of it on TNF-α,hs-CRP and plaque.To explore the effect of it against inflammation and stabilizing plaque.To provide evidence-based medical evidence for Xiaorong Wenban Fang to treat unstable angina pectoris.Methods:60 patients were collected and divided into 2 groups randomly,that is,the control group and the treatment group,with 30 cases in each group.The control group was treated with conventional western medicine such as antiplatelet,anticoagulation,antiischemic.ThestatinuseRosuvastatinCalcium Tablets（10mg/d）.The treatment group was treated with Xiaorong Wenban Fang in addition to conventional western medicine.To observe the changes of TCM syndrome score,TCM syndrome efficacy,serum lipid and serum inflammatory cytokines TNF-α,hs-CRP level and coronary atherosclerotic plaque in two groups of patients.The statistical analysis of the data obtained by the SPSS19.0 software package is carried out and the conclusion is drawn.Results:1.The difference of sex,age and course of disease of the two groups was not statistically significant（P>0.05）,and it was comparable.2.Before treatment,the difference between the two groups was not statistically significant（P>0.05）,which was comparable.3.After treatment,the two groups were improved in angina,the dosage of nitroglycerin,TCM syndrome score and TCM symptom curative effect.The treatment group was better than the control group in the improvement of angina,TCM syndrome score and TCM syndrome（P<0.05）.There was no significant difference between the two groups in the rate of reduction of nitroglycerin（P>0.05）.4.After treatment,the serum levels of TC,TG,LDL-C,TNF-αand hs-CRP were lower than those before treatment（P<0.05）,and the treatment group was better than the control group（P<0.05）.5.After treatment,the number of non calcified plaques in the two groups was less than that before treatment,but the difference was not statistically significant（P>0.05）.The number of non calcified plaques in the treatment group decreased significantly compared with that before treatment（P<0.01）.The number of mixed plaquesin the treatment group increased after treatment（P<0.05）.The number of calcified plaques in the treatment group increased after treatment,but there was no statistical significance（P>0.05）.After treatment,the non calcified plaque in the control group decreased compared with that before treatment（P<0.05）,and the number of calcified plaques and mixed plaques increased,but the difference was not statistically significant（P>0.05）.Conclusion:Xiaorong Wenban Fang can improve the clinical symptoms of unstable angina pectoris,reduce the level of serum lipids and inflammatory factors TNF-and hs-CRP,thereby playing the role of anti-inflammatory and improving plaque.Experiment two:Effect of Xiaorong Wenban Fang on the expression of AMPK protein in human umbilical vein endothelial cells with ox-LDL interventionObject:The effect of Xiaorong Wenban Fang on the expression of AMPK protein in human umbilical vein endothelial cells after ox-LDL model was observed.To provide a theoretical basis for further exploring the mechanism of Xiaorong Wenban Fang in the treatment of unstable angina.Methods:With Xiaorong Wenban Fang and Rosuvastatin Calcium Tablets to normal male SD rats by gavage,then preparation containing serum.The human umbilical vein endothelial cells were divided into 6 groups,including the blank group,the model group,the rosuvastatin group,the low dose Xiaorong Wenban Fang group,the middle dose group of Xiaorong Wenban Fang and the high dose group of Xiaorong Wenban Fang.The blank group:10%normal rat serum+90%DMEM medium;the model group:ox-LDL（100mg/L）+10%normal rat serum+90%DMEM medium;the rosuvastatin group:ox-LDL（100mg/L）+10%rosuvastatin rats serum+90%DMEMmedium;thelowdoseXiaorongWenbanFang group:ox-LDL（100mg/L）+10%Xiaorong Wenban Fang low dose rat serum+90%DMEMmedium;themiddledosegroupofXiaorongWenban Fang:ox-LDL（100mg/L）+10%Xiaorong Wenban Fang middle dose rat serum+90%DMEMmedium;thehighdosegroupofXiaorongWenban Fang:ox-LDL（100mg/L）+10%Xiaorong Wenban Fang high dose rat serum+90%DMEM medium.After adding the medicine,the cells were placed in the incubator of 37 and 5%CO₂ for 24h.The expression of AMPK protein in human umbilical vein endothelial cells was detected by Western blot.The statistical analysis of the experimental data was carried out by the SPSS19.0 software package,and the conclusion was drawn.Results:Compared with the blank group,the phosphorylation level of AMPK in model group,rosuvastatin group,low concentration group of Xiaorong Wenban Fang and middle concentration group of Xiaorong Wenban Fang were all decreased（P<0.05）.The phosphorylation level of AMPK in high concentration group of Xiaorong Wenban Fang had a decreasing trend,but P>0.05 had no statistical difference.Compared with the model group,the level of AMPK phosphorylation in the rosuvastatin group,the low concentration group of Xiaorong Wenban Fang,the middle concentration group of Chinese medicine and the high concentration group of traditional Xiaorong Wenban Fan gall increased（P<0.05）.Compared with rosuvastatin group,the phosphorylation level of AMPK in low concentration group was lower than that in rosuvastatin group（P<0.05）,but there was no significant difference in AMPK phosphorylation level between Xiaorong Wenban Fang concentration group and Xiaorong Wenban Fang high concentration group（P>0.05）.Conclusion:Xiaorong Wenban Fang can promote the AMPK phosphorylation of human umbilical vein endothelial cells,thereby inhibiting the inflammatory response and improving the stability of atherosclerotic plaques.It provides a new idea for further exploring the mechanism of Xiaorong Wenban Fang.
