Objective:To investigate the clinical effect of microsurgical treatment of the fourth ventricular tumors was involved.Methods: 52 cases were admitted the fourth ventricle tumor from January 2014 to January 2017 of our hospital were analyzed retrospectively,including 4 cases by cerebellar vermis approach,34 cases by transcerebella medulla oblongata approach and 14 cases were combined approach.According to whether the patients with obstructive hydrocephalus were divided before operation.Group A,with different degrees of obstructive hydrocephalus;Group B,without obstructive hydrocephalus.Analysed and compared the two groups of patients’ KPS Scale(Karnofsky)after one week.Results: 44 cases(84.6%)underwent total resection and 8 cases(15.4%)underwent subtotal resection.The total remission rate of intracranial hypertension was 84.2%.The remission rate of dizziness was 85.3%,the remission rate of vomit was about 94.4%,the remission rate of vision abating was 0%,the remission rate of dysphagia was 33.3%,and the remission rate of ataxia was 75%.Postoperative KPS score results in a week:the prognosis of patients in group B were better than those in group A.The difference was statistically significant(P < 0.05).40 cases were followed up,and tumor recurrence occurred in 2 cases,death in 1 cases.Conclusions: In order to improve the effect of surgical treatment of the fourth ventricle tumor,the appropriate surgical approach selected,good surgical technique and effective treatment of complications were important.Hydrocephalus before operation has some influence on the prognosis. |
