Study On The Evaluation Method Of Clinical Curative Effect Of TCM Comprehensive Treatment For Chronic Obstructive Pulmonary Disease In Stable Period

[Purpose]To select and classify the evaluation index and methods of COPD through TCM therapy by using literature surveys,statistical analysis,expert consultation,etc.Moreover,scientific and practical evaluation is also involved to choose the relevant evaluation index more scientifically and reflect the efficacy of TCM therapy more objectively and more factually,thus the methodological reference can be provided to the TCM therapy in the stable phrase of COPD.[Methods]Search relevant reports of the past 10 years on COPD in existing TCM or TCM-WM magazines and sort out evaluation index and methods of COPD;Employ five respiratory clinical TCM or VM specialists from top three hospitals inside and outside Hunan Province to assess(centesimal system)the importance of each research element(main symptom evaluation method,physical sign assessment method,comprehensive assessment of symptoms and signs,quality of life,lung function,etc.)Make further efforts to count and analyze the significance of research elements,mainly examining the quality of each element from multiple perspectives of significance,sensibility,representativeness,etc.Specifically,methods like percentage,factor analysis,subjective evaluation are employed to give a comprehensive analysis to the research elements.Finally,based on the comprehensive analysis,an objective,scientific and practical clinical evaluation method for the comprehensive treatment of COPD in TCM is refined and summarized to improve the scientific evaluation of clinical efficacy of COPD.[Results]The results of literature researchThe evaluation of curative effect of TCM treatment of COPD in stable period in most of the laboratory index,quality of life,symptoms and signs of the comprehensive evaluation method(symptoms with one or several items of laboratory evaluation),part of the research on traditional Chinese medicine syndrome score,few studies have been used in order to outcome indicators(such as survival time,frequency of acute attack,other indicators(such as mortality),BODE index,6min walking test)were evaluated.The results of experts consultationThe efficacy evaluation of TCM comprehensive treatment in the stable phase of COPD should be based on the quality of life,recurrence rate or other endpoint evaluation or long-term efficacy indicators,combined with clinical symptoms and signs and necessary laboratory indicators(such as lung function)to evaluate,at the same time,the selection of evaluation indicators should be based on the different needs of clinical research.[Conclusion]At present,the efficacy index of TCM treatment of COPD in the stable phrase is mainly based on comprehensive evaluation of symptoms and signs,evaluation of laboratory indicators,evaluation of quality of life,and evaluation of recurrence rate.For clinical application,comprehensive analysis should be conducted based on the differences on types of study or treatment,stage of disease or evaluation period,outpatient or inpatient,and objective and scientific evaluation indicators should be selected.
