Objective: Based on the school,the oral health comprehensive intervention project was carried out in school-age children,including oral health education,oral hygiene guidance and local application of fluoride varnish due to evaluate the anti-caries effects of first permanent molar and explore the compensation mechanism of children’s oral health comprehensive intervention model.Methods: During 2014.09 to 2016.12,2003 Children aged 6 ~ 8 were selected from Dahua County by random sampling method for 2-year oral health comprehensive intervention.Due to the loss of samples,eventually 1793 people were included in the statistics analysis.These samples were randomly divided into experimental group and control group.The children in the experimental group were treated with fluoride vanish twice a year in addition with oral health education and oral hygiene guidance,whereas the control group was only provided with oral health education and oral hygiene guidance.The data was collected by Oral Health Examination and Questionnaire.The results wererecorded and analyzed to evaluate the anti-caries effects of fluoride on the first permanent molars.Specific interventions were as follows:Carrying out oral health education: Researchers used 10 to 20 minutes teaching students about basic knowledge of oral care,how to develop healthy eating habits and oral hygiene habits and then used dental models to demonstrate the method of BASS brushing.Oral health guidance: Students were provided a set of fluoride toothpaste and toothbrush and instructed on-side by the researchers to brush their teeth,in order to correct their bad brushing method and ensure that their oral hygiene is in good condition before oral examination.Local application of fluoride: After the oral examination,the first permanent molars were coated with standard amount of Duraphat after their tooth surfaces were dried.The remaining amount of Duraphat was applied on other remaining teeth.The children were warned not to drink water,eat foods for 2 hours and not to brush teeth during the same day.Results: As for the ratio of children eating desserts and candy,sweet drinks,sweetened drinks 1 or more times a day,there was no statistically significant difference between the two groups before or after the intervention(P > 0.05).As for the ratio of children brushing 2 or more times a day,using toothpaste,using fluoride toothpaste,there was no statistically significant difference between the two groups before or after the intervention(P > 0.05).Before the intervention,the first permanent molar prevalence rate of experimental group was 24.11%,DMFT was 0.45,DMFS was 0.59,while the control group was 23.20%,0.43,0.53 respectively.There was no statistically significant difference between the two groups(P > 0.05).After the intervention,the prevalence rate and DMFS of experimental group were 54.17%,1.69,lowerthan 59.37% and 1.98 of control group.There was statistical significant difference between the two groups(P <0.05).After the intervention,the first permanent molar incidence was 30.06%,the newly added DMFS was 1.10,which was lower than the control group(36.17%,1.45),There was statistical significant difference between the two groups(P<0.05).After the intervention,the incidence of occlusal surface,buccal surface,palatal surface,mesial surface,bistal surface,pit and fissure,proximal caries incidence was 18.46%,5.84%,2.94%,0.45%,0.82%,27.24% and 1.26%respectively,which were lower than those of 24.14%,8.31%,5.26%,1.58%,2.01%,37.72% and 3.58% presented in control group.There was statistical significant difference between the two groups(P <0.05).Evaluation of anti-caries effects: Caries reduce relatively effective rate was13.26%.Conclusion: This research shows that the through implementation of comprehensive intervention measures,experimental group caries reduce relatively effective rate is 13.26%.It suggests that carrying out oral health education,oral hygiene guidance,local application of fluoride could effectively prevent the happening of the first permanent molar caries in the primary school.Intervention measures has high popularization value,due to it is easy to be implemented,...
