| Objective:To explore the significance of the thickness of lipid layer in the diagnosis and meibomian gland function evaluation of dry eye patients with meibomian gland dysfunction,and to explore the correlation between the thickness of tear lipid and the proportion of incomplete blink.Methods:Collect Shandong Eye Hospital from January 2017 to January 2018 a total of 54 cases(108 eyes)accepted the relevant examination subjects of sex,age,break-up time(BUT),Schirmer I test(Schiemer I),meibomian gland function evaluation score(MGE),the relevant inspection data,ocular surface interferometer LipiView include: average the thickness of the lipid layer of the tear film(AvgICU),the maximum thickness of the lipid layer(Max ICU),the minimum thickness of the lipid layer(MinICU),distribution(Max ICU-Min ICU),incomplete blink ratio(PB).Were grouped according to the following criteria: 1.meibomian gland dysfunction group inclusion criteria:(1)16 years of age or older,gender and race is not restricted;(2)BUT 5S or Schirmer I≤5mm/5min;(3)a dryness,foreign body sensation,burning sensation,fatigue,discomfort,visual fluctuation etc.one of the subjective symptoms;(4)slit lamp examination are as follows: the change of eyelid eyelid hyperemia;the palpebral margin irregular,thickened or obtuse;the meibomian gland orifice occlusion or fat embolus;the Marx line shift.2.control group inclusion criteria:(1)16 years of age or older,gender and race is not restricted;(2)without eye discomfort.Statistical analysis of the two,comparison of meibomian gland dysfunction group and control group,the average thickness of the lipid layer lipid distribution balance,not blinking whether there is difference ratio,explore the tear lipid layer thickness and the correlation between the proportion of incomplete blinking,the correlation between tear lipid layer thickness and meibomian gland function score,tear film distribution and the correlation between the proportion of incomplete blinking.An independent sample t test was used for the comparative analysis of statistical data.Pearson bilateral test and general linear model estimation were used for the correlation test.Results:Among the subjects,there were 20 males(37.0%)and 34 females(63.0%)with an average age of 36.11±11.51 years.There were 35 cases(64.8%)in the meibomian gland dysfunction group: 14 cases(40.0%)in males and 21 cases(60.0%)in females,mean age 37.34±12.41 years,meibomian gland function score(MGE)was17.71±6.15 points,mean lipid layer thickness(AvgICU)was 47.40±12.76 nm,maximum lipid layer thickness(Max ICU)was 59.71±12.67 nm The minimum lipid layer thickness(MinICU)was 38.06±12.65 nm,the distribution(Max ICU-Min ICU)was 21.66±9.45 nm,the incomplete blink(PB)was 0.53±0.31,and the control group was 19(35.2%).There were 6 males(31.6%)and 13 females(68.4%),with an average age of 33.84±9.52 years,and meibomian gland function score(MGE)46.79±8.74.Lipid layer thickness(AvgICU)58.58±9.09 nm,Max ICU 68.53±9.22 nm,MinICU 45.66±12.95 nm,the distribution(Max ICU-Min ICU)22.87±9.46 nm,incomplete blink percentage(PB)ratio 0.26±0.31.Statistical analysis: There was a statistically significant decrease in lipid layer average thickness in the meibomian gland dysfunction group compared with the control group(t=3.375,P=0.00140<0.05);lipid distribution of tear film in the meibomian gland dysfunction group was lower than that in the control group.The difference in balance was not statistically significant(t=0.450,P=0.655>0.05);the ratio of incomplete eyelids in the meibomian gland dysfunction group was statistically significantly higher than that in the control group(t=2.917,P=0.00521<0.05);tear fluid fat The negative correlation between mass thickness and incomplete blinking was statistically significant(Pearson correlation coefficient=-0.286,P=0.0357<0.05),and the negative linear relationship between the two linear models(b=-10.940,a=46.573);according to the average There was a statistically significant negative relationship between lipid layer thickness and MGE scores(Pearson correlation coefficien,t=0.285,P=P=0.00367<0.05).The negative linear relationship between the two was linear.The model(b= 0.352,a=9.867);there is a positive relationship between the distribution of tear film distribution and the proportion of incomplete blinking(Pearson correlation coefficient=-0.300,P = 0.0277 <0.05),and the negative linear relationship between the two.Linear model(b=-0.011,a=0.670).Conclusion:The thickness of the lipid layer can be used as an important auxiliary diagnosis for dry eye patients with meibomian gland dysfunction.The thickness of the lipid layer is important for the assessment of meibomian gland function.There is a negative relationship between the proportion of partial blink and the thickness and distribution of the tear film lipid layer. |
