| Objective:To investigate the clinical efficacy.of intravitreal injection of anti-vascular endothelial growth factor(VEGF)drugs combined with Ahmed glaucoma valve in the treatment of neovascular glaucoma(NVG).Methods:Fifty-two patients(52 eyes)with neovascular glaucoma who underwent AGV implantation from January 2015 to May 2017 were enrolled.Whether intravitreal injections of anti-VEGF drugs were given 3-10 days before surgery In the injection group(group A,n=28)and non-injection group(group B,n=24),All patients were followed up for 1 day(1d),1 week(1Wk),1 month(1Mo),3 months(3Mo)and 6 months(6Mo).The neovascularization and recurrence,intraocular pressure control,best corrected visual acuity,number of antiglaucoma medications,success rate of surgery,and complications were compared between the two groups.Results:1.Compared with the non-injected group,93%(26 eyes)of neovascularization on the iris surface of the injection group completely resolved within 3 to 10 days after injection of the anti-VEGF drug,but no regression was found in the non-injected group.2.Compared with preoperative,postoperative follow-up at all time points,the intraocular pressure was significantly lower in both groups,with statistical differences(P<0.05);Compared with the non-injected group,the mean intraocular pressure of the injection group was lower at each follow-up time point,and there was a statistically significant difference(P<0.05).3.Visual acuity increased 53.6%and 14.3%in the injection and non-injection groups after treatment,46.4%and 53.6%in the eyes without visual acuity,visual acuity decreased in 0%and 17.9%,and visual acuity improved in the injection group compared with preoperative There was a statistically significant difference(P<0.05).There was no significant difference in visual acuity between the non-injection group and the non-injection group(P>0.05).Compared with the non-injection group,the visual acuity improved in the injection group,with statistical significance(P<0.05).).4.Compared with preoperative,the number of drugs used to reduce intraocular pressure after surgery was significantly lower in the two groups(P<0.05).Compared with the non-injected group,the number of drugs used in the injection group was Id,lWk,and IMo after surgery.There was a statistically significant difference between the two groups(P<0.05).At the remaining follow-up periods,the use of the two groups was similar,with no statistical difference(P>0.05).5.The success rates of the patients in the injection group were 96.4%,96.4%,92.9%,89.2%,and 85.7%,respectively.The non-injection group was 54.1%,58.3%,66.7%,62.5%,and 58.3%,respectively.Compared with the non-injected group,the success rate of the injection group was higher,with statistical differences(P<0.05).6.The incidence of hyphema in the injection group and non-injection group was 7.1%and 58.3%,postoperative drainage tube obstruction was 0%and 8.3%,and the formation of filtration blebs were 7.1%and 37.5%,respectively.All had statistically significant differences(P<0.05);postoperative shallow anterior chambers were 21.4%and 20.1%,and choroidal hemorrhage or detachment were 3.6%and 4.1%,respectively,with no statistical difference(P>0.05).Conclusion:1.The intravitreal injection of anti-VEGF drugs before neovascular glaucoma Ahmed glaucoma valve implantation can quickly eliminate neovascularization and create conditions for the next surgical treatment.2.Intravitreal injection of anti-VEGF drugs in combination with Ahmed glaucoma glaucoma valve implantation for the treatment of neovascular glaucoma can better control intraocular pressure,reduce the number of anti-glaucoma drugs used,improve the visual acuity and surgical success rate,reduce complications,is a treatment one of the safe and effective methods for neovascular glaucoma. |
