| Clinical research Objective: To observe the clinical effect of Yangxinshi tablets on the stable coronary artery disease(QI deficiency and blood stasis syndrome)with sleep disorder and on the content of lipid and glucose,and provide the clinical basis for using Yangxinshi tablets to treat stable coronary heart disease with sleep disorder.Methods: 60 patients with stable coronary heart disease and sleep disorder diagnosed by Western medicine,meanwhile diagnosed by traditional chinese medicine(TCM)with Qi deficiency and blood stasis were randomly divided into two groups,labeled as treatment group and control group,respectively.Each group was 30 cases.The two groups were given lifestyle intervention and regular basic medicine treatment.The treatment group was additionally given Yangxinshi tablets.After 4 weeks of treatment,two groups were observed in patients with TCM syndrome score,efficacy of TCM syndromes,score of anginal intensity,efficacy of angina pectoris,the content of triglyceride(TG),cholesterol(TC),high density lipoprotein(HDL-C),low density lipoprotein(LDL-C),fasting blood glucose(FPG),postprandial 2 hours blood glucose(2h PG),glycosylated hemoglobin(Hb Alc),the score of Pittsburgh sleep quality index(PSQI),the change of sleep disorders.Finally,the collected data are processed and analyzed by the relevant statistics.Results: 1.Before treatment,two groups baseline data(gender,age distribution,average course,complication,vascular,scaffold,blood lipids,glucose,left ventricular ejection fraction,Pittsburg Sleep quality index score)had no significant difference.2.After treatment,the TCM syndromes of the treatment group decrease significantly compare with the control group(P<0.05),the therapeutic effect of TCM syndromes is better than control group.3.After treatment,the score of anginal intensity in both groups are decreased P<0.01),the treatment group decrease more significant P<0.05),and the total effective rate of angina pectoris was higher than control group P<0.05).4.After treatment,the content of TG,TC and HDL-C of both groups are improved.In the aspect of lower TC and LDL-C,the different of two groups had no significant difference.However,in the aspect of lower TG and LDL-C,the different of two groups had significant difference.5.After treatment,he blood glucose level of both groups decreased,but the different not exist statistical significance.6.After treatment,the PSQI score in the treatment group exist statistically significant P<0.01)compare with the control group,the treatment group was more effective in reducing the score of PSQI P<0.01),and the treatment group is better than the control group in improving the efficacy of sleep disorder.Conclusion: Yangxinshi tablets can improve the clinical and accompanying symptom of patients with stable coronary heart disease(Qi deficiency and blood stasis)combined with sleep disorder,also can reduce the level of TC,LDL-C and PSQI integral effectively.Animal Experiment Study Objective: To observe the effect of Yangxinshi tablets on myocardial energetic metabolism in rats with acute myocardial ischemia.Methods: Sixty standard rats were divided into five groups randomly,12 in each group,namely,high dose group of Yangxinshi tablets,low dose group of Yangxinshi tablets,Trimetazidine group,model group,sham operation group.Among the high and low dose group,Trimetazidine group and model group,the rats of myocardial ischemia were induced by coronary ligation,and the model rat of acute myocardial ischemia was prepared.The sham operation group had no ligation of the needle.After the mold is successful,the drug is poured into the stomach for 14 days after first 24 hours.The drug is poured into the stomach for 14 days.The pathological changes of serum creatine kinase(CK-MB),cardiac troponin T(c Tn T)and the results of 14-day gastric lavage were detected in rats after the 24 th hour of rat model,and the detection of CK-MB,c Tn T,Free fatty acid(FFA),Glycogen(Gly),succinate dehydrogenase(SDH),creatine phosphate(CP),adenosine triphosphate(ATP)content,NA+-K+-ATP enzyme activity,protein expression of adenosine activator protein kinase(AMPK α2)and Ampk-m RNA expression level in 14thdays(it means experiment finished).To explore the drug effects of myocardial energetic metabolism in rats with acute myocardial ischemia.Results: Compared with the sham operation group,the secum content of CK-MB and c Tn T in model group is increased,the level of FFA also increase(p<0.05),the content of SDH and Gly decrease,CP and ATP decrease,and NA+-K+-ATP enzyme activity decrease.The expression level of AMPKα2 protein and AMPK-m RNA decrease(p<0.05);Compare with the model group,Yangxinshi tablets can imrove the pathology change of ischemic myocardium,decrease the content of CK-MB,c Tn T,FFA(p<0.05),increase the content of SDH,Gly and CP,NA+-K+-ATP enzyme activity,increase the level of AMPK α2protein expression and Ampk-m RNA expression(p<0.05).Conclusion: Yangxinshi tablets may improve myocardial ischemia and its energetic metabolism by changing the indexes and mechanisms of myocardial ischemia and myocardial energetic metabolism in acute myocardial ischemia. |
