Objectives To investigate the effect of extracorporeal shock wave on the healing of BTJ tendon bone after reconstruction of rabbit Achilles tendon.Methods Bilateral Achilles tendon injury repair models were established in New Zealand white rabbits.The right sides were treated with shock wave,while the left one without any intervention.4 weeks and 8 weeks after operation,the sections of tendon bone healed were observed by HE staining.The expression level of TGF-β1 and bFGF mRNA was measured by PCR,and biomechanical testing was performed.Results The HE staining showed that the tissue healing and inflammatory cell reduction in shock wave treatment group were faster than those in control group.The expression of bFGF in shock wave treatment group at 4 weeks and 8 weeks was significantly higher than that in control group(p <0.05).There is no difference between the4 week’s and 8 week’s expression respectively.The expression of TGF-β1 in the treatment group after 4 weeks and 8 weeks after operation was significantly higher than that in the control group(p <0.05).There is no difference between the 4 week’s and 8 week’s expression respectively.Biomechanical test showed that the maximum load of separation of Achilles tendon from calcaneus was 35.39 ± 7.15 N,61.12 ± 4.47 N in the control group at the fourth week and the eighth week after the Achilles tendon reconstruction,while the treatment group was 65.98±9.02 N、97.23±5.94 N respectively.The maximal stretching distance of the Achilles tendon was 1.47 ± 0.10 cm and 1.77 ± 0.08 cm at the 4th and 8th week respectively in the control group,while the treatment group was 1.83±0.06 cm and1.94±0.07 cm respectively.There was a significant difference between the maximum load in two groups,as well as the maximum stretch distance in each time period(p <0.05),and the maximum load and the maximum stretching distance at different time points within each group were significantly different(p < 0.05).Conclusions Extracorporeal shock wave intervention group was better than control group in tensile properties of Achilles tendon-bone junction,tendon-bone interface maturation,and promotion of tendon bone healing growth factor content,indicating that extracorporeal shock wave can promote the process of BTJ tendon bone healing and improve its strength and ultimate load. |
