Objective:1.Compare the variation trend of drug used to control cancer pain after the implementation of Good Pain Management Ward Program.Evaluate whether the cancer pain management of this hospital meet the requirements of GPM.2.Select 63 patients of esophageal cancer as research object to compare effect of standardized cancer pain management on the quality of life and survival,to discuss clinical significance of standardized cancer pain management.Methods:1.Query clinical medicine management system of this hospital,summarize the dosage and composition changes of cancer pain management drug.2.Query electronic medical records?related records of Pain treatment and follow-up records of patients,eliminate cases which have incomplete records,then divide these cases into two groups according to the result of cancer pain treatment normative evaluation.One group is named standardized treatment group,the other is therapy owe norms group.Compare the differences of Pain Management Index?QLQ C30 and QLQ-OES18 after cancer pain therapy,analysis the effect of standardized cancer pain management on survival.Results:1.After the implementation of Good Pain Management Ward Program,the variation trend of drug used to control cancer pain,cancer pain assessment?Adverse Drug Reaction(ADR)monitoring meet the requirements of GPM.2.After three weeks of cancer pain treatment,the PMI result of 80%patients in standardized group is shown effective,and with the extension of time,cancer pain treatment effectiveness is gradually improved.Compared with therapy owe norms group,the difference has statistically significant.3.Results of QLQ-C30 survey:patients in standardized treatment group got higher score in the functional areas of the body function,emotional function and the overall situation,the difference between two groups has statistically significant(P<0.05).In symptom field,patients in standardized treatment group got relieved on the symptom of pain,but the condition of patients in therapy owe norms group was not improved,the difference between two groups has statistically significant.Both of the two groups got a higher score on fatigue,nausea,vomiting,insomnia,loss of appetite,constipation,after cancer pain treatment.Result of QLQ-OES18:score of dry mouth and loss of appetite increased,the difference of two groups has no statistically significant,this kind of situation may be associated with the side effects of opioids which were used to control cancer pain.4.Effect of standardized cancer pain treatment on survival:estimate average survival period of patients in standardized treatment group is 33.034,and the result in therapy owe norms group is 18.024 months,the difference between two groups has statistically significant(P=0.016<0.05),by further analysis,tumor metastasis or not is the confounding factor that affect the survival time of patients.Conclusion:standardized cancer pain management can improve the effectiveness of clinical cancer pain treatment.It is also beneficial to improve the quality of life in patients with esophageal cancer,especially in improving patients' physical function,emotional function and overall health condition,impact on patients' survival still needs further clinical study.
