Status Of Depression,Coping Strategy And Hope In Patients With Stroke And Their Relationship

Objective(1)To understand the status of the hope level,depression and coping styles of general hospitalized patients with stroke.(2)Through investigation,analysis the influence factors of the hope level of patients wiht stroke.between the hope level,depression and coping styles of inpatients with stroke.(3)To analysis the correlation analysis between the hope level,depression and coping styles of inpatients with stroke.To provide theoretical reference basis of effective intervention measures to improve the stroke patients' hope level.MethodsThis study is a descriptive survey.By cluster random sampling,the study select 180 cases of stoke patients which is accordance with the inclusive criteria and exclusive criteria during December 2015 to October2016 in two comprehensive hospitals of Hunan province.The patients were investigated by self-made general data questionnaire Herth Hope Inder Scale(HHI),Self-rating depression scale(SDS)and Medical Coping Modes Questionnaire(MCMQ).The Epidate3.1 software is used to process the data and the SPSS18.0 statistical software is used to analyze the data.Statistical methods include descriptive statistics analysis,t-Test,single factor analysis of variance,multiple linear stepwise regression,Pearson and Spearman related analysis,etc.Results(1)The average score of hope of stoke patient was(33.30±5.11),the scoreof per-dimension from high to low is take positive action(11.36 ±1.71),keep close relationship with others(11.06±1.98),Positive attitude towards the future and reality(10.88±2.08);(2)Univariate analysis shows that different gender,age,level of education,occupation,marriage condition,working conditions,place of residence,living conditions,family per capita income,medical payment,self-care ability,stroke frequency and stroke types of stroke patients' hope level is different,the difference is statistically significant(P < 0.05);Multiple step wise regression analysis shows that living conditions,the types of stroke,stroke frequency,level of education,family per capita income,self-care ability are put into the regression equation,the fitting multivariate regression equation is statistically significant(F = 15.136,P < 0.001);(3)The total scores of depression of stoke patients is(57.11±12.34),with no depression is accounted for 37.2%,mild depression is 20.0%,moderate depression is 22.2%,severe depression is 20.6%.The score of thethree dimensions medical coping styles,face of coping styles is(18.78 ±3.99),avoidance coping style is(17.32 ±2.89),yield response is(10.38±3.98);(4)Pearson correlation analysis results show that the level of depression scores is negatively correlated with the total score of hope level(r = 0.760,P < 0.01).Spearman correlation analysis showed that: in the face of dealing were positively correlated with the total score of hope level and its three dimensions(positive attitude,positive action,keep close relationship with others)(r = 0.500,r = 0.426,r = 0.371,r = 0.463,P <0.01),avoidance response were positively correlated with the total score of hope level and its all dimensions(r = 0.260,r = 0.194,r = 0.217,r =0.211,P < 0.05 or P < 0.01);Depression score showed a negative correlation with the three dimension of hope level(r =–0.603,r=–0.502,r =–0.703,P < 0.01),the yield response showed a negative correlation with hope level and all its dimensions(r =–0.640,r =–0.640,r=–0.378,r =–0.606,P < 0.01)Conclusions(1)The level of hope in patients with stroke is medium level,compared to the others is low.(2)Most of the patients with stroke have depression with different degrees.Most patients used avoidance and yield response,less use the face of coping styles.(3)Multiple stepwise regression analysis results show that the living situation,the type of stroke,stroke frequency,level of education,family per capita income,self-care ability are the main factors that affect the level of hope with stroke,the living situation is the most main factors affect the cerebral apoplexy patients.(4)The hope level of patients with stroke and its all dimensions is positive correlated with face up coping and avoidance coping;the hope level and its all dimensions is negative correlation with yield copying and the depression.It is to say that the more the patient is use the face up coping,the higher its level of hope;the patients with more yield copying and with higher depression,the level of hope will be lower.
