Objective : Investigating adiponectin concentrations in patients with ankylosing spondylitis(AS),compared with healthy controls and analyze whether adiponectin is correlated with clinical indexes and TNF-α of AS patients.Method:Fifty AS patients from Department of Rheumatism and Immunity,the First Affiliated Hospital of beng bu medical college were included in this study.They all fulfilled the modified New York diagnostic criteria for AS.Tirty matched healthy control with age and sex-matched were enrolled.Serum levels of adiponectin and TNF-α were measured with enzyme immunoassay methods.The clinical activity was estimated using the functional score Bath Ankylosing Spondylitis Functional Index(BASFI)and the Bath Ankylosing Spondylitis Disease Activity Index(BASDAI).Clinical information were collected in all the patients at the time of the study,including duration、weight and height、total cholesterol(TC)、triglycerides(TG)、HDL and LDL cholesterol、c-reactive protein(CRP)、erythrocyte sedimentation rate(ESR)、radiographic grading and other clinical data were assessed.Sacroiliac joint CT grading was less than grade II defined as early stage and more than grade II defined as advanced stage.According to whether hip involvement or not divided into two groups.Results :1.Comparison of serum adiponectin level and TNF-α in two groups:adiponectin levels did not differ between AS patients and controls[(5.16±1.83)vs(4.79±1.13)ug/m L],while serum TNF-α level in AS patients was higher than controls[(16.31±5.77)vs(12.23±3.10)pg/m L].2.Adiponectin levels and TNF-α were significantly elevated in AS patients with hip involvement compared with patients without hip involvement(P<0.05).Adiponectin levels was no statistically significant difference between early stage and advanced stage,but serum TNF-α level in AS of early stage patients was lower than advanced stage patients.3.In 50 AS patients,serum adiponectin levels was positive correlation with CRP(r=0.321,P=0.041)and negative correlation with BMI(r=-0.606、P=0.000) and TG(r=-0.340、P=0.035).No correlation was found between adiponectin and TC 、HDL 、LDL、ESR、BASDAI 、BASFI、TNF-α.In 27 patients with hip involvement,serum adiponectin levels correlated significantly with CRP(r=0.55,P=0.006)、BASDAI(r=0.429、P=0.023),no significant association was found between adiponectin and ESR、TNF-α(P(29)0.05)。Conclusions:1.The serum levels of adiponectin in patients with hip involvement are higher than in patients without hip involvement.2.The serum levels of adiponectin in patients with hip involvement positively correlate with CRP and BASDAI,it suggests that adiponectin may contribute to the occurrence of hip lesions in ankylosing spondylitis.3.No significant association is found between adiponectin and TNF-α.
