Objectives To observe the changes of serum levels of tumor necrosis factor(TNF-α)and interleukin-6(IL-6)in patients with ankylosing spondylitis,and to analyze the influence of TCM syndrome types and disease characteristics on them,so as to provide reference for the temporal treatment of ankylosing spondylitis.Methods From March 2019 to December 2019,60 hospitalized patients in the Rheumatology department of Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei Province who met the aspiration standards were selected AS the experimental group(AS group).In the same period,15 healthy physical examinees in our hospital were selected as control group.Physical examination on admission determined whether there was peripheral joint involvement.The experimental group was divided into AS activity group and remission group by Bath ankylosing spondylitis disease Activity index.Four diagnostic methods of traditional Chinese Medicine and the primary and secondary syndromes in traditional Chinese Medicine were used to classify the experimental group.The sacroiliac joint X-ray was performed for imaging classification.The venous blood of subjects from the two groups were collected from the period of the day from 11 p.m to 1 a.m,period of the day from 5 a.m to 7 a.m,period of the day from 11 a.m to 1 p.m and period of the day from 5 p.m to 7 p.m.The serum levels of TNF-α and IL-6 were measured by chemiluminescence method.The diurnal changes of subjects were observed and the effects of TCM syndrome types and disease characteristics on them were analyzed.SPSS21.0 statistical software was used for data analysis and processing,and P<0.05 was considered statistically significant.Results 1 The serum levels of TNF-α and IL-6 in the normal control group showed no statistically significant difference at different times of day and night(P>0.05),and the level fluctuations were relatively stable and within the normal range,with no significant diurnal changes.The serum levels of TNF-α and IL-6 in the experimental group(AS group)were significantly different at different times of day and night(P<0.05),which were significantly higher than those in the normal control group,reached the peak at the period of the day from 11 pm to 1 am and the valley piont at the period of the day from 11 am to 1 pm.2The levels of TNF-α and IL-6 in the AS active group and the remission group were different at different times of day and night,with statistical significance(P<0.05).The levels of TNF-α and IL-6 in the AS active group reached the peak at the period of the day from 11 pm to 1 am and the valley point at the period of the day from 11 am to 1 pm.,and the levels of TNF-α and IL-6 in the AS catabasis group reached the peak at the period of the day from 5 am to 7 am and the valley at the period of the day from 11 am to 1 pm.3The levels of TNF-α and IL-6 in the serum of different TCM syndromes were different at different times of day and night,with statistical significance(P<0.05).The serum TNF-αreached its peak at the period of the day from 11 pm to 1 am when the syndrome of dampness-heat obstruction and the syndrome of cold dampness obstruction reached the valley at the period of the day from 11 am to 1 pm.In the syndrome of blood stasis obstruction and syndrome of kidney deficiency and governor cold,Deficiency syndrome of liver and kidney,The serum IL-6 level reached its peak value in syndrome of dampnessheat obstruction,syndrome of cold dampness obstruction,and syndrome of blood stasis obstruction at the period of the day from 5 am to 7 am.,and reached its valley value at at the period of the day from 11 am to 1 pm,when the syndrome of kidney deficiency andgovernor cold,Deficiency syndrome of liver and kidney.4 different image classification AS the serum TNF-α,IL-6 level is different hour in the day and night,with statistical significance(P<0.05),that is,both in unilateral Ⅲ,bilateral Ⅱ peak at the period of the day from 11 pm to 1 am.to valley at the period of the day from 11 am to 1 pm;In unilateral Ⅳ,bilateral Ⅲ peak at the period of the day from 5 am to 7 am,valley at the period of the day from 11 am to 1 pm.5 The serum levels of TNF-α and IL-6 with or without peripheral joint involvement were significantly different at different times of day and night(P<0.05),When the peripheral joints were involved,they reached the peak at the period of the day from 11 pm to 1 am and the valley at the period of the day from 11 am to1 pm.In the absence of peripheral joint involvement,both of them reached peak value at the period of the day from 5 am to 7 am and valley at the period of the day from 11 am to 1pm.Conclusions 1 There were diurnal changes in serum levels of TNF-α and IL-6 in patients with AS,with the the period of the day from 11 pm to 1 am(peak)and the period of the day from 11 am to 1 pm(valley).2 The diurnal changes of serum TNF-α and IL6 levels in patients with AS were different
