| Objective:Objective to explore the effectiveness of health education based on IMB skill model to improve compliance of COPD patients with exercise rehabilitation and its effect on cognition,exercise endurance and quality of life of COPD patients.Methods:(1)This study was a quasi experimental study from May 2017 to December,to facilitate a province to a three level of first-class hospital department of respiration in accordance with the inclusion criteria of 92 cases of hospitalized patients with COPD,according to a group of respiratory ward group,intervention group,two respiratory group as control group,intervention group and control group of 46 cases.(2)The intervention group received health education model based on IMB techniques,respectively,on the day of admission,a week of hospitalization and discharge at three time points,were distributed and explain the COPD booklet,motivational interviewing,explanation and demonstration of breathing and physical exercise skills;after discharge telephone follow-up 1 times a week,and was discharged from the hospital4 after the 1 week follow-up guidance.The patients in the control group received routine health education during hospitalization and followed up for 1/2 weeks after discharge.2months in all patients at baseline and after discharge,the knowledge,attitude and behavior(KAP)questionnaire,compliance questionnaire,6 minutes walking distance(6MWD)and COPD evaluation test scale(CAT),were evaluated KAP levels,exerciserehabilitation compliance,exercise endurance and quality of life in two groups with the application,Chi square test,t test and rank test were used to analyze the data.Results:(1)A total of 86 patients completed this study,42 cases in the intervention group and 44 cases in the control group.The general data of the two groups had no statistical significance(P>0.05),and had comparability.(2)There was no significant difference between the scores of KAP total scores and the scores of each dimension in the two groups before intervention(P>0.05).2 months after discharge,two patients with KAP were higher than those before intervention(P<0.001),and the patients in the intervention group KAP scores were higher than the control group,the difference was statistically significant(P<0.05);two groups of patients before and after intervention KAP score difference between the two groups,the intervention group scores were higher than the control group,the difference was statistically significant(P<0.001).(3)There was no statistical difference between the two groups before the intervention(P>0.05).2 months after discharge,the intervention group compliance score higher than that before the intervention,the difference was statistically significant(P<0.001),the control group and the compliance score before the intervention,the difference was not statistically significant(P>0.05);2 months after discharge,the compliance of the intervention group were higher than that of control group,the difference was statistically significant(P<0.001);two groups of patients before and after intervention compliance score difference between the two groups,the intervention group score changes was higher than the control group,the difference was statistically significant(P<0.05).(4)There was no significant difference in 6MWD comparison between the two groups before intervention(P>0.05).2 months after discharge,two patients in 6MWD group were increased compared with before intervention(P<0.001),and 6MWD in theintervention group was higher than that of the control group,the difference was statistically significant(P<0.05);two groups of patients before and after 6MWD intervention the difference between two groups,the intervention group to improve the degree of 6MWD was higher than the control group,the difference was statistically significant(P<0.001).(5)There was no significant difference between the scores of CAT total scores and the scores of each item in the two groups before intervention(P>0.05).2 months after discharge,two groups of patients with CAT score and each item score compared with before intervention,the differences were statistically significant(P<0.05),compared two groups of patients with CAT score and each item score between the groups,in addition to sleep,the rest of the intervention group CAT total score and each item score was lower than the control group,the differences were statistically significant(P<0.05);two groups of patients before and after intervention CAT score and each item score difference between the two groups,in addition to sleep,the intervention group scores were higher than the control group,the difference was statistically significant(P<0.05).Conclusion:(1)Health education based on IMB skill model can effectively improve COPD patients’ exercise compliance,enhance their cognition level of disease,improve exercise endurance and improve their quality of life.(2)The health education based on IMB skill model is applied to the effectiveness of sports rehabilitation management in COPD patients,so as to provide new thinking for clinical nurses in sports rehabilitation management of COPD patients. |
