Objective:To investigate the effect of transcranial direct current stimulation(tDCS)combined with hyperbaric oxygen(HBO)therapy on the patients with delayed encephalopathy after carbon monoxide poisoning(DEACMP).Methods:1.A total of 40 patients who were diagnosed with DEACMP at the Second Hospital of Hebei Medical University from December 2016 to February 2018 were selected.They were randomly divided into treatment group(20 cases)and control group(20 cases).Both groups of patients were given hyperbaric oxygen therapy(HBOT),conventional individualized rehabilitation therapy and medications for improvement of microcirculation and nutritional nerves,The treatment group was treated by tDCS based on conventional therapy.All treatments for tDCS,HBOT,and conventional individualized rehabilitation therapy were performed once a day,6 days per week,for 3 courses.One course of treatment is ten days.Before and after 3 courses of treatment,the cognitive function of patients was assessed using the Mini-Mental State Examination(MMSE)and P300.Barthel index(BI)was used to analyze the daily life of patients(activities of daily living,ADL).2.Measurement data were expressed as mean ± standard deviation,The difference was statistically significant according to P<0.05.Results: Before treatment,the basic conditions(age,gender,education level,latency phase,the time from illness to viste)of the two groups were not statistically different.There was no significant difference between the treatment group and the control group in MMSE score(treatment group = 6 ± 5,control group = 4.8 ± 3.7)and BI score(treatment group = 29 ± 16,control group = 20 ± 15),P>0.05.P300 latency(treatment group = 372 ± 10 ms,control group = 379 ± 14ms)and amplitude(treatment group = 4.3 ±2.3μV,control group = 3.3 ± 1.8μV)showed no significant difference(P >0.05).After 3 courses of treatment,the MMSE and BI scores of the treatment group and the control group were both higher than those before treatment.The latency of P300 in the two groups was significantly shorter than before treatment,and the amplitude was increased.The difference was statistically significant(P<0.05).After treatment,compared with the control group,the MMSE score of the treatment group(13±7)was higher than that of the control group(9±5)(P<0.05).The BI score of the treatment group(54 ± 17)was higher than that of the control group(43 ± 15)(P <0.05),P300 incubation period(342 ± 29)ms in treatment group was shorter than that in control group(363 ±17)ms,amplitude(7.0 ± 3.3)μV was higher than that in control group(5.1 ± 2.7)μV.Statistically significant(P <0.05).Conclusion:Transcranial direct current stimulation(t DCS)combined with hyperbaric oxygen(HBO)can help improve cognitive function and ADL of DEACMP patients.This combination therapy might be a helpful method to enhance recovery of patients with DEACMP. |
