| ObjectiveTo compare the differences of pregnancy complications and perinatal outcomes between monochorionic and dichorionic twin pregnancies,so that clinicians should pay attention to the diagnosis of chorion and amnion,so as to take timely intervention measures to improve the survival of twin pregnancy.MethodIn this study,274 cases of definite chorionicity during the period from January to December in 2016 were studied.These subjects were obstetrics and gynecology at the Affiliated Hospital of Qingdao University.The study used a retrospective case-control study.According to the chorionicity of the study subjects,they were divided into monochorionic diamnionic twin(MCDA group)68 cases and dichorionic diamnionic(DCDA group)206 cases.The clinical data of these subjects were collected to analyze the differences in maternal,fetal,and neonatal outcomes.Results1.The impact of chorionicity on the maternal: The HDCP,GDM,placenta praevia,placental abruption,ROPM,hypothyroidism,postpartum hemorrhage,anemia,cervical dysfunction etc are insignificant between monochorionic and dichorionic twin pregnancies.There were significant differences in the incidence of premature birth,ICP and hydramnion.2.The impact of chorionicity on the fetal: The fetal complications including single intrauterine fetal death,twin-twin transfusion syndrome,discordant growth twin,fetal malformation,fetal distress,abnormal umbilical blood flow.The differences of them are insignificance.3.The impact of chorionicity on the neonatal: The neonatal complications contain GA,premature birth,NRDS,respiratory failure,neonatal hypoglycemia,NEC,anemia,hypothyroidism,coagulation dysfunction,neonatal pneumonia,cholestasis,BPD,ROP and hyperbilirubinemia etc are insignificant between monochorionic and dichorionic twin pregnancies.Among them,BW,LBW,NICU admission,neonatal asphyxia,neonatal brain injury,neonatal hypoglycemia and congenital heart disease were significantly different.Conclusion1.There were significant differences in the incidence of premature birth,ICP and hydramnion between monochorionic and dichorionic twin pregnancies2.Among the monochorionic and dichorionic twin pregnancies,BW,LBW,NICU admission,neonatal asphyxia,neonatal brain injury,neonatal hypoglycemia and congenital heart disease were significantly different.3.The key to clinicians to improving the prognosis of perinatal children is clearing chorionicity and strengthening the monitor and intervention of monochorionic diamnionic twin. |
