Objective Type 2 diabetes is a clinically common disease that seriously jeopardizes people’s health and reduces the quality of life and even life-threatening.It is a group of metabolic diseases characterized by chronic hyperglycemia caused by multiple causes,which are caused by insulin secretion and/or metabolism.Caused by defects,can cause a variety of serious complications such as heart,brain,kidney and blood vessels.With the improvement of living standards,the incidence of diabetes is increasing year by year.How to control blood glucose and diabetes has become an urgent problem to be solved.Currently,there are many kinds of oral hypoglycemic drugs.The traditional insulin secretagogues have the side effects of causing hypoglycemia and weight gain,which is unfavorable to the management of diabetic patients.Nowadays,new types of hypoglycemic drugs have gradually entered the public eye,including DPP-4 inhibitors and SGLT2 inhibitors,and their effects on body weight,body fat composition,and blood glucose control need to be proved by clinical trials.Neuregulin 4(Nrg4)is a neuregulin,and brown adipose tissue can be secreted in small amounts.Studies have shown that Nrg4 can increase insulin sensitivity and improve blood lipid profile.At present,there are no studies on the effects of SGLT2 inhibitors and DPP-4 inhibitors on Nrg4.In this study,the serum Nrg4 levels and body fat composition were measured in diabetic patients after applying dapagliflozin and saxagliptin.Method A total of 43 patients with Type 2 Diabetes admitted to the Department of Endocrinology at the Eastern Hospital Affiliated Hospital of Qingdao University from August 2016 to December 2017 were randomly selected into 2 groups and treated with dapagliflozin(n=20).The program: the original hypoglycemic regimen based on the use of dapagliflozin 10 mg qd + metformin 500 mg tid;saxagliptin group(n = 23)treatment program: the original application of hypoglycemic base saxagliptin 5mg qd + metformin 500 mg tid;Followed up for 8 weeks.At baseline and at 8 weeks of treatment,body mass(BW),body mass index(BMI),glycosylated hemoglobin(Hb A1c),blood lipids(TG,TC,HDL,LDL),uric acid(UA),and serum neuregulin 4(observed)were observed.Nrg4)level,body fat composition measurement of body fat composition changes.Results(1)After 8 weeks of treatment,the body weight,BMI,Hb A1 c,total body subcutaneous fat,visceral fat,and body fat after treatment in the dapagliflozin group were lower than before treatment(P < 0.05).There was an increase before treatment(P<0.05).The basal metabolic rate,triglyceride,cholesterol,HDL,LDL,and uric acid did not change significantly after treatment(P>0.05).(2)Body weight,BMI,total body subcutaneous fat,whole body muscle,visceral fat,body fat,basal metabolic rate,triglyceride,cholesterol,HDL,LDL,and uric acid after treatment with saxagliptin No significant change(P>0.05).After treatment with glucoammoglobin,there was a decrease in glycosylated hemoglobin(P<0.05).(3)There was no significant change in Nrg4 after treatment with dapagliflozin and saxagliptin compared with before treatment(P>0.05).(4)Analysis of the two groups before treatment showed that the body fat percentage(r=0.305)was related to Nrg4.Using Nrg4 as a dependent variable,BMI,Hb A1 c,total body subcutaneous fat rate,whole body muscle ratio,visceral fat content,body fat percentage,basal metabolic rate,triglyceride,cholesterol,high density lipoprotein,low density lipoprotein,uric acid were Independent variables were analyzed by multiple stepwise linear regression analysis.The results showed that only body fat percentage and BMI were included in the regression equation,and were independent influence factors of Nrg4(β was 0.403,0.310,and P values were 0.011 and 0.046,respectively).The change of Nrg4 before and after treatment was used as the dependent variable,and the change of each index was taken as independent variable.Multiple linear linear regression analysis was performed.The results showed that only uric acid was included in the regression equation(β=0.307,P=0.045).Conclusion Dalgliflozin has the effects of reducing body weight,reducing body fat content,and effectively reducing glycosylated hemoglobin,but has no significant effect on blood lipids,uric acid,and the like.The saxagliptin can effectively reduce the glycated hemoglobin,but no significant effect on body weight and blood lipids.Regarding Nrg4 factor,there was no significant effect of both drugs.Meaning This article analyzes the effects of weight,glycosylated hemoglobin,body fat,and Nrg4 before and after treatment in the two groups,and analyzes the effect on the related index.It was found that Dalaglipc hypoglycemic has the effect of reducing body weight and reducing body fat content,and finds application 8 Both groups had no significant effect on Nrg4 after the week.This provides guidance for the selection of clinical medications and provides ideas for other clinical or animal experiments. |
