| Objectives:The aim of this study was to investigate the pathological characteristics of primary biliary cirrhosis(PBC)and its overlapping syndrome(AIH).To explore the relationship between the autoantibodies of TGF-β1 and IL-17)and the primary biliary cirrhosis,autoimmune hepatitis and its overlap syndrome,to provide experimental basis for the molecular pathological mechanism of autoimmune liver disease.Methods:The subjects of this study were 52 patients with autoimmune liver disease diagnosed in Chengyang District people’s Hospital of Qingdao from May 2016 to May 2017,including 15 patients with AIH26 and 15 patients with AIH-PBC OS 11.The clinical manifestations and liver function of the above patients were compared and analyzed.Liver histological characteristics and immunoglobulin level,TGF-β1 IL-17 levels in AIH-PBC OS3 group and control group were detected by immunohistochemical method,andANA、SMA、AMA、IgM、IgG、IgA was detected by fluorescence method.The concentration of 12 TGF-β1 IL-17 was detected by ELISA.Result:1.Female for autoimmune liver disease high incidence of groups,there are more than diabetes or other autoimmune diseases complications;AIH patients with first symptoms mostly loss of appetite,fatigue,abdominal discomfort and fever,and PBC patients with skin itching as a typical performance.2.The levels of immunoglobulin G(IgG),serum alanine aminotransferase(ALT)and aspartate aminotransferase(AST)in AIH patients were significantly higher than those in patients with PBC,while y-glutamyltransferase(GGT),alkaline phosphatase)And immunoglobulin M(IgM)were significantly lower.AIH-PBC OS patients with ALP and transaminase were increased,IgG and IgM levels were improved.3.Autoimmune liver disease patients>90%anti-nuclear antibody(ANA)positive,AIH-PBC OS and PBC patients with anti-mitochondrial antibody(AMA)or AMA-M2 the majority of positive.4.AIH pathology mainly for the arrangement of liver cells and interstitial hepatitis,PBC patients with granulomatous cholangitis or non-suppurative injury cholangitis;AIH-PBC OS patients with varying degrees and PBC or AIH pathological changes.5.The expression of TGF-β1 in the liver tissue of the autoimmune liver disease group was significantly higher than that in the control group(P<0.05).The expression of TGF-β1 in the liver tissue of the autoimmune liver disease group was significantly higher than that in the control group But there was no significant difference between the three groups(P>0.05).IL-17 was mainly expressed in the liver tissue mononuclear cells of patients with autoimmune liver disease,and the portal area of inflammatory cells was mainly exposed to the main expression area.IL-17 expression was negative in the control group.Conclusion:1.The results of this study confirmed that 90%of patients with liver disease were positive for anti-nuclear antibody(Ana)and AIH-PBC OS was positive for AMA-M2 in patients with PBC.Serum IgG and IgM levels were increased.Therefore,humoral immune disorder is the main cause of autoimmune liver disease.2.TGF-β1 secreted by Treg cells was highly expressed in inflammatory and fibrosis regions,and the abnormal expression of TGF-β1 was closely related to the degree of hepatic fibrosis.3.IL-17 secreted by Th17 cells in patients with autoimmune liver disease is mainly expressed in monocytes of liver tissue of patients with autoimmune liver disease,and inflammatory cells infiltrate the catchment area,which can aggravate the inflammation of liver and lead to irreversible damage of liver cells.4.The imbalance of IL17 and TGF-β1 secreted by Th17 and Treg is an important molecular mechanism of the disease.This study provides a theoretical basis for the pathogenesis of autoimmune liver disease and opens up a new idea for the targeted treatment of autoimmune liver disease. |
