Objective:To investigate the risk factors of pancreatic pseudocysts(PPC)in patients with acute pancreatitis(AP)in a retrospective cohort study,and to provide reference for the prediction and treatment of pseudocysts after acute pancreatitis.Methods:460 AP patients with complete data admitted in Affiliated Hospital of Qingdao University from January 2010 to January 2017 were retrospectively analyzed,who were divided into case group(with pseudocysts)and control group(without pseudocysts).Gender,age,etiology,body mass index(BMI),history of diabetes,the presence of ascites and pleural effusion,the presence of palpable abdominal mass,CT severity index(CTSI),APACHE Ⅱ score in 48 h admission,neutrophil,serum albumin,lactate dehydrogenase,alkaline phosphatase,amylase,calcium,urea nitrogen,serum creatinine,triglycerides,total bilirubin,direct bilirubin,C-reactive protein were compared between the two groups.SPSS 23.0 statistical software was used for analysis.Univariate analysis was used to select the factors that were statistically different between two groups,and multivariate logistic regression analysis was performed to determine the independent risk factors of PPC after AP.Results:460 patients with AP were included in this study,345 were male and 115 were female.The mean age of the study population was 41.3±15.8 years.143(31.1%)patients developed a pancreatic pseudocyst,and the mean size of the pseudocysts was 42.2±21.3millimeters in diameter.On univariate analysis,factors including gender,etiology,CT severity index,body mass index,APACHE II score,history of diabetes,pleural effusion,ascites,palpable abdominal mass,serum albumin were statistically differentbetween two groups(P < 0.05).On Logistic regression analysis,it was shown that male sex(P <0.001,OR=17.571,95%CI=6.329~48.780),CTSI≥7(P=0.043,OR=3.695,95%CI=1.043~13.086),history of diabetes(P=0.047,OR=2.735,95%CI=1.016~7.364),ascites(P<0.001,OR=7.584,95%CI=3.139~18.321),pleural effusion(P=0.048,OR=2.554,95%CI = 1.010~6.461),a palpable abdominal mass(P = 0.026,OR = 4.193,95%CI =1.192~14.755)were independent risk factors significantly associated with the pancreatic pseudocysts formation in acute pancreatitis.Conclusions:The male sex,diabetic history,ascites,pleural effusion,palpable abdominal mass and CTSI≥7 were the independent risk factors of PPC formation in AP.Therefore,to prevent the formation of pancreatic pseudocysts and improve the prognosis,patients with acute pancreatitis who were observed the above factors should be taken seriously.
