Objective: To evaluate the clinical diagnosis and treatment of the pancreatic pseudocysts resulted from severe acute pancreatitis. Methods: A retrospective analysis of the clinical data was carried out on 69 patients with pancreatic pseudocysts who were undergone therapy in our hospital during february 2002 to February 2009. They were divided into external drainage group and internal drainage group according to different surgery Results: The exact rate of type-B ultrasonic is 84.1% and that of CT examination is 87.0%.After different treatment of 69 patients, 15patients were dead, 6 patients with secondary operation, 7 patients with postoperative heamorrhage, 25 patients with postoperative pancreatic fistula and 12 patients with postoperative infection.Eight patients of the surgical external drainage were dead, 4 patients with secondary operation, 8 patients with postoperative infection, 6 patients with postoperative heamorrhage, and 22 patients with postoperative pancreatic fistula. Two patients of the surgical internal drainage were dead, 2 patients with secondary operation, 3 patients with postoperative infection, 1 patients with postoperative heamorrhage, and 3 patients with postoperative pancreatic fistula. There is no statistical sense between the two groups about rate of secondary operation, postoperative infection, postoperative heamorrhage and mortality. There is significant statistical sense between the two groups about postoperative pancreatic fistula. Conclusions: Pancreatic pseudocyst resulted from severe acute pancreatitis is a serious disease and it had a high mortality. We should choose a correctly method for the patients on the basic of patient's condition, the time, diameter, state of an illness and the complications of the pseudocyst. We should give them conservative treatment on the early stage of the disease, and we will give them surgical treatment if it fails.
