Chronic obstructive pulmonary disease(COPD)is a common respiratory disease characterized by chronic cough,sputum and shortness of breath.However,for different patients,the severity of the disease is different,in clinical work,we often used to classify the severity of air flow restriction,the modified British Medical Research Council(m MRC)questionnaire,and the COPD Assessment Test(CAT)to assess the severity of the disease,we will be classified into non-COPD pneumoconiosis and pneumoconiosis.The correlation analysis of hemoglobin(Hb),lung function,blood gas analysis and so on was carried out to see if hemoglobin could evaluate the condition of patients with emphysema phenotype.Objective:The patients with COPD were classified into emphysema type and non-emphysema type by HRCT,and the significance of hemoglobin in the evaluation of pulmonary emphysema phenotype was evaluated by correlation analysis of hemoglobin(Hb),lung function and blood gas analysis.Methods:A total of 204 patients with stable chronic obstructive pulmonary disease were admitted to the Department of Respiratory Medicine of Jilin University Hospital from June 2016 to June 2017.According to the results of chest HRCT,patients with COPD were divided into emphysema group and non-emphysema group,and 45 healthy examiners were selected as control group.The correlation analysis of hemoglobin(Hb),lung function and blood gas analysis was carried out.Results:1.There was no significant difference in gender and age between the COPD group and the healthy control group(P>0.05),but compared with control group,There were more smokers in COPD group than non-smokers,and the smoking index of smokers was high.In COPD group,Pa CO2 and hemoglobin were higher than those in healthy control group,FEV1/FVC,FEV1%pred,and Pa O2 were lower than those in healthy control group(P<0.05),and there was no significant difference in PH between the two groups(P > 0.05).2.All patients had no correlation with FEV1/FVC,Pa O2,and negative correlation with FEV1%pred(R=-0.23,P=0.001);Positive correlation with Pa CO2(R=0.26,P=0.000);Negative correlation with PH(R=-0.34,P=0.000).3.There was no significant difference in age between the emphysema group and the non-emphysema group(P>0.05),However,compared with non-emphysema group,more men than women in emphysema group,more smokers than non-smokers,and smokers smoking index is high,the difference is statistically significant(P<0.05).Haemoglobin in emphysema group was higher than that in non-emphysema group,FEV1/FVC and FEV1%pred were lower than that in non-emphysema group,and the difference was significant(P<0.05).Pa O2 in emphysema group was lower than that in non-emphysema group,and Pa CO2 was higher than that in non-emphysema group.4.Hemoglobin was negatively correlated with FEV1%pred(R=-0.27,P=0.003),positively correlated with Pa CO2(R=0.41,P=0.000),negatively correlated with PH(R=-0.34,P= 0.000),and not with FEV1/FVC,Pa O2.5.Hemoglobin was negatively correlated with PH(R=-0.35,P=0.001)in COPD patients in non-emphysema group,but not with Pa O2,Pa CO2,FEV1/FVC,FEV1% pred.6.All patients with COPD,emphysema and non-emphysema had no significant correlation with smoking index.Conclusion:1.Hemoglobin can evaluate the condition of patients with pulmonary emphysema phenotype during COPD stabilization to some extent.2.Compared with non-pulmonary emphysema type patients,the lung function and blood gas analysis index of COPD phenotype patients in stable period should be paid attention to in clinical work,and reasonable and standard treatment plan should be formulated.
