Objectives:Hip dysplasia in adults as a hip joint fertility deformity is a common type of hip disease,developed to middle and late often secondary to severe osteoarthritis,seriously affecting the quality of life of patients.Total hip arthroplasty is an important treatment for DDH secondary to osteoarthritis of the hip(OA).The aim of this study was to investigate the clinical significance of reconstruction of hip joint center(Hip Joint Center,HCJ)during THA surgery.With the development of medical technology in our country and the aging population,the number of patients with DDH secondary to severe osteoarthritis increases year by year.DDH patients with acetabular deformity caused long-term abnormal biomechanical changes,resulting in the surrounding soft tissue contracture,stress concentration,severe cartilage damage,can cause severe hip dysfunction,joint deformity,claudication and pain,etc.seriously affected Quality of life of patients.Therefore,patients with secondary osteoarthritis later often require surgical reconstruction of hip function.THA is the best choice for the treatment of late-stage osteoarthritis of DDH.Intraoperative recovery of the normal biomechanical structure of the hip is the key to affecting the healing of joint function.The current study found that acetabular rotation center reconstruction may be one of the important components of the restoration of normal mechanical structure,and has an important impact on postoperative hip joint function recovery and improvement of quality of life of patients.The study was to investigate the effects of THA on the treatment of secondary osteoarthritis of DDH,the revival of revolving center and the early postoperative hip function and the quality of life in patients with DDH.Methods:Thirty-nine adult patients with hip dysplasia(DDH)secondary to severe osteoarthritis were selected,From October 2015 to August 2017,he received total hip arthroplasty in the Department of Joint Surgery,the Second Affiliated Hospital of Dalian Medical University.A total of 44 cases of hip replacement surgery,including 24 cases of left hip and right hip in 20 cases;including 4 males and 35 females.All patients received complete postoperative follow-up data.The follow-up time ranged from 6 to24 months(average 15.4 months).The Harris and SF-36 subscales were scored before and during follow-up.Postoperative measurement of prosthesis rotation center through the tear drop medial to establish a coordinate system.Postoperative and preoperative measurement of the rotation center of the template less than 5mm within the rotation as the center of the hip reconstruction,divided into rotational center reconstruction group and non-reconstruction group.Preoperative and postoperative SF-36 quality of life scores and Harris scores were compared between the two groups.Results:The patients with DDH secondary to osteoarthritis treated with THA had a preoperative Harris score of 60.22 ± 5.37 and a follow-up of 87.22 ± 3.77,P = 0.02<0.05.There was a statistically significant difference between preoperative and postoperative Harris scores.In the revolving center reconstruction group(24 cases),the excellent and good rate was 91%;In the non-reconstruction group(20 cases),the excellent and good rate was 60%.Pearson chi-square test,there was a statistically significant difference between the two groups(P <0.05).SF-36 quality of life score before surgery was 647 ± 49.68,postoperative 712 ± 46.56,after testing P <0.05,the score was statistically significant.SF-36 quality of life scale analysis of reconstructed and non-reconstructed groups between groups,statistical tests p> 0.05,no significant difference.Conclusions:1.THA is an effective surgical treatment of DDH secondary to severe osteoarthritis.2.DDH secondary to osteoarthritis THA treatment,acetabular rotation center anatomical reconstruction is conducive to improving postoperative hip function and improve quality of life.
