Objective: We analyzed related factors affecting the prognosis of pancreatic cancer to explore the clinical effects of integrated traditional Chinese medicine and western medicine(TCM-WM)on it.Methods: 80 patients with pancreatic cancer were diagnosed and admitted to Dalian Hospital of Traditional Chinese Medicine and the First Affiliated Hospital of Dalian Medical University from January 2012 to January 2016,and all were retrospectively analyzed and followed up by telephone,clinic and hospitalization except that 8 patients were lost to follow-up among these patients.We employed univariate analysis to analyze the patient’s gender,age,history of diabetes,primary tumor sites,liver metastasis,KPS score,CA19-9,surgical treatment,radiotherapy,chemotherapy and TCM treatment effects.The variables with statistical significance were selected as the dependent variables of the Cox regression model,and then the effects of these indicators on the survival time of patients with pancreatic cancer treated with integrated traditional Chinese and western medicine were analyzed.In addition,the KPS score scale was used to compare the scores of 68 patients with pancreatic cancer treated with traditional Chinese medicine before and after treatment,and later the effects of integrated treatment of traditional Chinese and western medicine on the survival quality of patients was evaluated.Results:(1)By analyzing the survival status of 72 patients with pancreatic cancer who completed the follow-up within 2 years,the data showed that there were 49 survivors in6 months and the survival rate was 61.25%;34 survivors in 12 months and the survival rate was 42.5%;18 survivors in 18 months,and the survival rate was 22.5%;and what’s more 11 survivors in 24 months and the survival rate was 13.8%.(2)Univariate analysis was performed to screen the clinical indicators related to survival of patients with pancreatic cancer,and the results showed that history of diabetes,primary tumor sites,liver metastases,KPS score,CA19-9,surgical treatment,chemotherapy and traditional Chinese medicine treatment were the factors that affected prognosis of pancreatic cancer(p<0.05).However,there was no statistical difference among age,gender and whether to receive radiotherapy for patients with pancreatic cancer(p>0.05).(3)Cox regression model multivariate analysis of the above indicators on the prognosis of patients with pancreatic cancer,the results showed that history of diabetes,the primary tumor site located at the head and neck of the pancreas,liver metastasis,KPS<60,CA199≥500U/ml were the risk factors affecting the survival time of patients with pancreatic cancer(p<0.05).In addition,surgical treatment,chemotherapy and traditional Chinese medicine treatment could prolong the survival time of pancreatic cancer patients(p<0.05).(4)All the selected 68 patients treated with traditional Chinese medicine were divided into only traditional Chinese medicine treatment group(31 cases),surgical treatment group(17 cases)and integrated chemotherapy and Chinese medicine treatment group(20 cases).By comparing the KPS score of the three groups of patients before and after treatment with Chinese medicine,we concluded that integrated traditional Chinese medicine and western medicine treatment was more advantageous to improve the survival quality of patients with pancreatic cancer(p< 0.05).Conclusions:(1)With the progression of pancreatic cancer,the survival rate of patients was gradually decreasing.(2)The history of diabetes,primary tumor sites of pancreatic cancer,CA19-9,and hepatic metastases were the most important clinical factors affecting the survival time of80 cases of pancreatic cancer.Besides,the history of diabetes,primary tumor site at head or neck of pancreas,CA19-9 ≥500U/ml,KPS<60,and hepatic metastasis were important factors affecting the prognosis of patients with pancreatic cancer.(3)Surgical treatment,chemotherapy,and Chinese medicine treatment could prolong the survival time of patients with pancreatic cancer,so these may be beneficial to improve the prognosis of patients with pancreatic cancer.(4)After surgical treatment and chemotherapy,traditional Chinese medicine treatment could obviously improve life quality of patients with pancreatic cancer.Thus,we make a conclusion that integrated traditional Chinese medicine and western medicine treatment has a potential value to treat pancreatic cancer. |
