Reasearch Of The Relationship Between Physician Professionalism And Professional Practice Environment

Objective Through the description and analysis of the basic situation of physicians in 9 hospitals in Liaoning Province,and the systematic investigation of the current situation of their professionalism and practicing environment,Explore the relationship between them and puts forward relevant countermeasures and suggestions in a targeted manner so as to provide references for managers to formulate corresponding policies.With a view to improving the practicing environment,the level of physicians professionalism and speeding up the development of occupational-related research.Methods 1.Literature review: Using databases such as CNKI,Wanfang Database,Pubmed,Web of Science and so on,to find domestic and foreign literatures related to the physicians professionalism,influence factors of physicians professionalism and the practice environment.Organize and summarize the important theories and views of literatures as the basis of this study.2.Epideminological survey: In this study,Penn State University Professionalism Scale and the first introduced to our country's Professional Practice Environment Scale survey of nine hospitals' physicians in Liaoning Province,in order to obtain the data needed for this study.3.Sampling method: Using stratified random sampling method.The first stage stratified sampling according to per capita GDP and geographic location to determine the city.The second stage according to the overall strength of the nine cities in eight cities selected.The third stage was to extract all physicians from 9 hospitals for investigation.4.Statistical Analysis: Use descriptive statistical analysis to understand the overall status of the sample,the issuance and recovery of questionnaires,the overall level of professionalism and practice environment.Univariate analysis was used to understand the differences in professionalism and practicing environment between different demographic characteristics,different GDP levels and different hospital categories.Use correlation analysis to explore the correlation between professionalism and the practice environment.Using multivariate linear regression analysis to determine whether the practice environment can affect the physician's professionalism and whether the various dimensions of the practice environment have an impact on the dimensions of physician professionalism and physician professionalism.Data analysis apply SPSS20.0 and AMOS24.0.Results A total of 389 questionnaires were distributed in the pre-investigation,369 valid questionnaires were collected,and the effective recovery rate was 94.8%.The Cronbach's alpha reliability coefficient of the Professional Practice Environment Scale was 0.942,and Spearman-Brown's half-coefficient was 0.905.After the items were cut and debugged,the relevant validity indexes of GFI,RMSEA,NFI,IFI,TLI,CFI,PGFI,PNFI and CMIN/DF all reached the standard.In the formal investigation,a total of 4343 questionnaires were sent out,and 4138 questionnaires were returned,with an effective quantity of 3899 and an effective rate of 89.78%.The physicians who were older,whose academic qualifications were lower and working years were longer had higher scores in the professionalism and practice environment,physicians in the internal medicine department and other departments had higher scores in the professionalism and practice environment,and both the scores of professionalism and practice environment in the female physicians were higher than male.Hospitals with lower per capita GDP in cities where hospitals are located have higher scores in professionalism and practicing environment.The scores of professionalism and practicing environment in affiliated hospitals are higher than those of non-affiliated hospitals.The correlation between professionalism and practice environment was significant,with a correlation coefficient of 0.80,p <0.001,showing a significant positive correlation.The dimensions of physicians professionalism and the dimensions of practicing environment are all related.The demographic characteristics,GDP and hospital category as the control variables.The result of multivariate linear regression analysis showed that the regression coefficient of professional environment to professionalism was 0.75,p <0.001,and the regression coefficient was statistically significant.The internal work included in the practice environment the regression coefficients of motivation,control practice,leadership and autonomy in clinical practice,relationship with physicians,teamwork,and communicate with patient were 0.88,0.42,0.50,0.32,0.54,p <0.001;Work motivation,leadership and autonomy in clinical practice,relationship with physicians,and communicate with patient with a predictable predictive role on professionalism,including the accountability,altruism,responsibility,excellence,honesty and integrity,respectful;control of the practice are predictive to honesty and integrity;teamwork has a predictive effect on the excellence,honesty and respect.Conclusions 1.The demographic characteristics,hospitals at different economic levels,universities affiliated hospitals and non-affiliated hospitals,the level of professionalism and practice environment in which the situation is different.2.The gender,educational level,working year,profession,where the hospital is located per capita GDP of the city,and the type of hospital will have a certain impact on the professionalism of the physicians.3.Physicians professionalism has a positive correlation with the practice environment.Internal motivation,control practice,leadership and autonomy in clinical practice,relationship with physicians,teamwork and communication with patients have a positive correlation with the professionalism.4.The practice environment has a positive predictive effect on the professionalism,and the higher the practice environment,the professionalism will be higher.5.Internal work motivation,leadership and autonomy in clinical practice,doctorpatient relationships,teamwork,and communicate with patient can positively predict physician professionalism.
