Objective:To investigate the incidence of anxiety and depression in patients with chronic heart failure and quality of life and other related factors.Methods:Baseline data of 102 consecutive patients with chronic heart failure(CHF)who were admitted to Zhongshan Hospital Affiliated to Dalian University between 2016.10-2017.9 and signed informed consent form were enrolled.To investigate the relationship between anxiety and depression status and quality of life in patients with chronic heart failure.Research content: demographic data: including sex,age;personal history: smoking history,drinking history,history of previous or combined cardiovascular disease: hypertension,diabetes,atrial fibrillation,coronary heart disease;The height,weight,neck circumference(NC)and waist circumference(WC)were measured.Myocardial injury markers were detected: c Tn I,lactate dehydrogenase(LDH);Heart function related evaluation index: NYHA classification,N-terminal B type natriuretic peptide(NT-pro BNP),6-minute walk test(6-MWT),left ventricular ejection fraction(LVEF);quality of life(quantitative assessment): minnesota living with heart failure questionnaire.MLHFQ score;Mental status assessment: Self-Rating Anxiety Scale(SAS)was used for anxiety status;Self-Rating Depression Scale(SDS)was used for depression status assessment.According to scores,anxiety and depression status were evaluated.As well as the severity;SPSS 22.0 software was used for statistical analysis.Results:The incidence of anxiety in patients with chronic heart failure was 4%,depression was 21.6%,the incidence of anxiety and depression in patients with chronic heart failure was 33.3%,heart failure without anxiety or depression was 41.1%;Our study showed that there was significant difference between anxiety and non-anxiety groups in variables: female,history of atrial fibrillation,history of diabetes,NYHA classification,NT-pro BNP,6-MWT(walking distance)and quality of life(P <0.05);Variables: Female,history of atrial fibrillation,history of diabetes,history of coronary heart disease,NYHA classification,NT-pro BNP,6-MWT(walking distance),quality of life(score),neck circumference were significantly different between depression and non-depression(P <0.05),There was no significant difference in age,smoking and drinking history,history of hypertension,troponin,LDH,and waist circumference between anxiety and non-anxiety,depression and non-depression groups(P>0.05).The analysis of the risk of anxiety and depression in patients with chronic heart failure by different variables showed that the risk of anxiety and depression in patients with chronic heart failure was significantly higher than that of women with atrial fibrillation,diabetes,NYHA class ? or ?,NT-pro BNP> 5000 ng / l,LVEF <45%,6-MWT(Moderate and severe)were the influencing factors of anxiety and depression in patients with chronic heart failure(P <0.05).Correlation analysis of anxiety,depression and chronic heart failure: Influencing factors were analyzed by multivariate logistic regression model.After correction,the history of atrial fibrillation,gender,and MLHFQ score were independently affected factors of anxiety in patients with chronic heart failure;diabetes history,MLHFQ Scoring is an independent risk factor for depressive symptoms in patients with chronic heart failure.Conclusions:The incidence of anxiety in patients with chronic heart failure was 4%,depression was 21.6%,the incidence of anxiety and depression was 33.3%,without anxiety or depression was 41.1%,Chronic heart failure patients with high incidence of anxiety and depression,History of atrial fibrillation,gender,and MLHFQ score were independent predictors of anxiety in patients with chronic heart failure.The history of diabetes mellitus and MLHFQ score are independent risk factors for depressive symptoms in patients with chronic heart failure.Therefore,we should pay attention to patients with chronic heart failure combined with anxiety and depression,should be identified as soon as possible,intervention,help to improve the quality of life in patients with chronic heart failure and improve prognosis.
