BackgroundDizziness is a kind of subjective feeling.It refers to the abnormal sensation of standing or walking instability,top-heavy,but there is no self or the motion of the object or the sense of rotation.In the department of neurology,more and more middle-aged and young patients with dizziness combined with abnormal cervical physiological curvature.Foreign team not only confirmed the abnormity cervical physiological curvature of vertebral basilar artery has internal pathogenic effects,but also found that the cervical curvature change and the back of the neck pain,dizziness,and lower quality of life.This topic focuses on the study of the relationship between them.We have taken a retrospective analysis of 78 cases of middle-aged and young dizziness,taken four segments of the cervical spine.According to the Borden measurement method,which were divided the cervical curvature abnormal group and cervical curvature of the normal group.Then according to the relevant literature,the cervical curvature changes were classified into three groups: the mild group,the medium group and the severe group.To explore whether there is a correlation between the dizziness of young and middle-aged patients and cervical physiological curvature changes,and to do the clinical diagnosis and treatment.ObjectiveTo explore whether there is a correlation between the dizziness of young and middle-aged patients and cervical physiological curvature changes.To analysis of risk factors for young and middle-aged patients with dizziness of the abnormal cervical vertebra physiological curvature.To investigate whether there is a correlation between the different dizziness types and the abnormal cervical vertebra physiological curvature in the middle and young people.Method(1)To use the retrospective analysis to collect young and middle-aged(18~55 years old)patients of dizziness in two neurology department of Third Affiliated Hospital of Xinxiang Medical College.The diagnosis of dizziness accords with the diagnostic procedure of dizziness.According to the clinical manifestations of Post and Dickerson in 2010,we divide the dizziness into 4 types: dizziness(lightheadedness),vertigo(vertigo),imbalance(disequilibrium)and presyncope(presyncope),the vertigo is the most common.(2)To collect detailed medical records of patients who were hospitalized due to dizziness,dizziness,dizziness,blurred vision,unstable walking and syncope as the main symptoms or complaints,and the clinical baseline data of the patient with no more than 24 hours of onset.After collecting the complete medical records,making the EXCEL form,The general baseline data of the names,sex,age,occupation and living habits of all selected patients were recorded in detail;detailed questions about past smoking,drinking history,history of hypertension,hyperlipidemia,diabetes history,hyperhomocysteinemia,past cerebrovascular disease history,past history of coronary heart disease and other related risk factors were detailed.The main onset forms,clinical manifestations and positive signs of the nervous system were recorded in detail,the ABCD2 score was used in all patients.All patients complete liver function,renal function,blood lipid,blood sugar,glycosylated hemoglobin,blood homocysteine,hypersensitivity C reactive protein and other hematological tests,and cranial MRI,magnetic resonance angiography(MRA),cervical vascular ultrasound,transcranial Doppler ultrasound(transcranial Doppler,TCD),cervical vertebrae four slice examination and so on.(3)For each selected patient,four slices of cervical vertebrae were taken.The Borden measurement was used to measure the physiological curvature of the cervical vertebrae in each patient,which was divided into the normal group of the cervical vertebra physiological curvature and the group of abnormal cervical curvature.According to the results of the measurement,the severity of the curvature of the cervical spine was divided into three groups.Mild abnormal cervical curvature group: measurement of cervical curvature,arc height is 1~7mm,with cervical curvature straight;moderate abnormal cervical curvature group: arc height of 1~-4mm,and accompanied by cervical anti bow,combined with a series of cervical vertebrae,severe abnormal cervical curvature group: arc height of less than-4mm obviously,cervical lordosis reverse,with cervical vertebral body and intervertebral space narrowing difference sequence anomaly.The results were evaluated by two deputy chief physicians and above senior physicians,and consulted with senior radiologists when necessary.(4)SPSSl9.0 statistical software package was used for processing data,and P<0.05 was considered that difference was statistically significant.The standard deviation(Standard Deviation,SD)was used for the measurement data,and the t test was used for the comparison between the groups.A single factor analysis of variance was used in multiple groups,and the two-two comparison between the two groups was carried out.The ratio of counting data was expressed,and the chi square C2 test was used among the groups.The correlation analysis was used to analyze the correlation between the severity of the cervical curvature change and the dizziness,and the scatter plot was plotted.To analyze the risk factors of young and middle-aged dizziness patients with cervical curvature,and to explore the correlation between cervical curvature and cervical dizziness.Result(1)A total of 78 young and middle-aged patients with dizziness is choosen.According to the change of the curvature of the cervical spine,it is divided into the normal group of cervix and the group of cervical curvature.Then according to the severity of cervical curvature abnormality,it was divided into mild cervical curvature abnormal group,moderate abnormal group and severe abnormal group.Among them,there were 56 cases(72%)of abnormal cervical curvature,24 men and 32 women,with an average age of(55.24+10.32)years.There were 22cases(28%)in the normal cervical curvature group,8 in male and 14 in female,with an average age of(52.42+6.68)years.There was no significant difference in age and gender between the two groups(P>0.05).There was no statistically significant difference in the history of hypertension,cerebral infarction and coronary heart disease between the two groups(P>0.05).(2)There were 56 cases of cervical curvature abnormalities in 78 cases of cervical vertebra physiological curvature change.In accordance with the severity of cervical curvature were divided into three groups: mild cervical curvature abnormality group 19 cases(1~7mm),moderate cervical curvature change group of 19 cases(1~-4mm),18 cases of severe cervical curvature change group(-4mm).(3)Hyperhomocy steinemia,diabetes,GLU,HbA1 c,Hcy and occupational factors in young and middle-aged dizziness patients with cervical curvature change were significantly higher than those in normal cervical spine group(P<0.05).The mental labor of occupational factors was significantly higher than that of manual labor(P<0.05).However,there was no significant difference between the MRI images and the color Doppler ultrasound in the patients with abnormal curvature of the cervical spine and the normal cervical curvature group(P>0.05).(4)The ABCD2 scores and dizziness symptom in the group of middle-aged and young people with dizziness and cervical curvature were higher than the ABCD2 score in the normal group of cervical curvaturein young and middle-aged patients with dizziness(P<0.05);As the abnormal curvature of the cervical spine is becoming more and more serious,the time of illness is becoming longer and longer,and the clinical symptoms of dizziness in the patients are becoming more and more serious.Conclusion(1)The incidence of abnormal physiological curvature of cervical vertebrae is higher in young and middle-aged patients with dizziness..(2)There is a significant correlation between the middle-aged and young dizziness patients and the changes of the cervical curvature.With the more and more severe cervical curvature,the duration of the symptoms and the clinical symptoms of the dizzy patients are becoming more and more serious. |
