Clinical Observation On Treatment Of Cerebral Edema After Acute Cerebral Infarction By Jiawei Wuling Powder

Objective:By comparing the change of(MDA)and superoxide dismutase(SOD),the scale of the National Institutes of Health Stroke Scale(NIHSS)and the edema volume of the head CT at different time points after acute massive cerebral infarction,we can understand the effect of Jiawei Wuling Powder on ischemic stroke in reducing the size of edema and free radicals,antioxidant capacity of brain tissue and recovery of neurological function.Furthermore,we can clarify the possible mechanism of reducing the size of edema.Methods:(1)we collected 62 patients of acute cerebral edema in the Hospital of Shanxi TCM university between June 2015 to 2016 in December who meet the criterion of inclusion and exclusion.Than we divided them into two groups by the table of random numbers.The first group is control group of 30 cases,the oher is treatment group of 32 cases.(2)we give the two group basic treatment of enteric-coated aspirin tablets 0.1g,1 times / d,oral;simvastatin tablets 20 mg,1 times / d,oral;intravenous medication were edaravone 30 mg,2 times / d;Ginkgo biloba extract 75 mg,1 times / d;20% mannitol 125 ml,q8h,intravenous infusion,even for 10 d.In addition,the treatment group was given modified Wuling powder,1 dose / d,morning and evening hours serice,10 d.(3)we collected the edema volume of the head CT、the values of MDA and SOD and NIHSS score at the begining of the treatment、after 5 days,after10 days.(4)All data were statistically analyzed by SPSS 17.0 statistical software.The results were expressed as mean ± standard deviation(± s).The comparison between groups was analyzed by one-way ANOVA.The t-test was used to compare the data.For the difference of P<0.05 was statistically significant.Results:(1)Comparison of CT edema volume: After 5 days of treatment,the edema volume of the treatment group was higher than that of the control group,the difference was not statistically significant(P> 0.05).After 10 days of treatment,the edema volume of the treatment group was lower than that of the control group and the difference was statistically significant(P <0.05).(2)Neurological changes: After 5 days of treatment,the NIHSS score of treatment group was higher than the control group but the difference was not statistically significant(P> 0.05);after 10 days of treatment,the NIHSS score of treatment group was lower than the control group and the difference was statistically(P <0.05).(3)Compared withthe control group after 5 days,the serum MDA in the treatment group was higher than that in the control group(P <0.05)and the serum levels SOD in the treatment group was lower than that in the control group(P> 0.05).After 10 days of treatment,the serum MDA in the treatment group was lower than that in the control group(P <0.05).The serum SOD of the treatment group was higher than that of the control group.The two differences were both statistically significant(P <0.05).Conclusion :(1)Jiawei Wuling powder can efficientiy reduce the size of edema in treatment of ischemic stroke.after the possible mechanism of brain edema to reduce calcium overload,free radical scavenging,improve the antioxidant capacity of the brain tissue.(2)The possible mechanism of Modified Wuling powder in improving the edema volume effectively could account for reducing the overload of calcium、scavenging free radical and imporving the antioxidant capacity of brain tissue.(3)Compared with the mannitol group,flavored Wuling powder has the advantages of long action time,moderate,non-toxic side effects,and suitable use for long-term.
