| Henoch-Schonlein purpura(HSP)is a common vascular allergic disease caused by the body allergic to some sensitizing substances,resulting in increased capillary fragility and permeability,resulting in purpura,mucosa and some Some organs bleeding.The disease more common in adolescents,men slightly more than the incidence of women,more incidence in spring and autumn.1-3 weeks before the onset,most patients have general malaise,fever,fatigue and upper respiratory tract and other prodromal symptoms,followed by a series of typical clinical manifestations,such as non-thrombocytopenic skin contactable purpura,joint swelling and pain,abdominal pain,blood in the stool and hematuria,proteinuria and so on.The duration of the disease is generally about 2 weeks,poor prognosis patients with renal involvement.In recent years,Th17 cells and related cytokines IL-17 have been found to play an important role in inflammatory diseases.However,the role of Th17 and IL-17 in the pathogenesis of HSP is unclear.Matrix metalloproteinases(MMPs)are essential enzymes for degradation of extracellular matrix,and are involved in the development and occurrence of many autoimmune diseases.MMP-13 is an important member of the MMPs family,but at local and overseas on the relationship between MMP-13 and HSP and the relationship between MMP-13 and IL-17 little research.In this study,the proportion of Th17 cells in peripheral blood of HSP,the concentration of IL-17 and MMP-13 in plasma and the expression of IL-17 in affected skin tissue were determined.The relationship between these cells and cytokines and HSP was analyzed to further understand the relationship between Thl7 cells and its cytokines in the pathogenesis of HSP and its clinical significance.Objective:To investigate the expression level of Th17 cells,IL-17,matrix metalloproteinase-13(MMP-13)in plasma and IL-17 in the skin lesions of patients with Henoch-Schonlein purpura(HSP),to explore the immunological pathogenesis of HSP.Methods:The ratio of Th17 cells in the peripheral blood of HSP group and healthy control group was detected by flow cytometry(FCM).The concentration of IL-17 and MMP-13 in plasma of the study subjects was detected by ELISA.The expression of IL-17 in the skin biopsies was stained by immunohistochemistry.Results:(1)The proportion of Th17 cells in the peripheral blood of HSP group was(1.21 ±0.59)%,which was significantly higher than of the control group(0.71±0.26)%(t=4.907,P<0.01).(2)The level of IL-17 and MMP-13 in plasma of HSP group was higher than those in the control group(26.16 ± 14.90)pg/ml and(11.55±4.40)ng/ml,respectively(64.58 ± 36.21)pg/ml and(17.57 ± 5.40)ng/ml(t=6.183,P<0.01 和 t=5.022,P<0.01).(3)The integral optical density of IL-17 in HSP group was(7.26 ± 2.34),which was higher than that in control group(4.61 ± 1.82)(t=2.877,p=0.009).Conclusion:Th17 cells,IL-17 and MMP-13 may be involved in the course of the immunological pathogenesis of Henoch-Schonlein purpura. |
