The Content Of Matrix Metalloproteinase In Children With Henoch-Schonlein Nephritis And Its Relationship With Renal Damage

OBJECTIVE:By detecting the content of the serum and urine matrix metalloproteinase-9(MMP-9),tissue inhibitor matrix metalloproteinase-1(TIMP -1) in the patients with Henoch-Schonlein purpura nephritis(HSPN),Henoch-schonlein purpura(HSP) and healthy children,and detecting 24-hour urine protein, serumβ2-MG,urineβ2-MG,serum creatinine and blood urea nitrogen (BUN) at the same time,our study investigate the role of MMP-9 and TIMP-1 in the patients with HSPN and the relationship with their renal damage。METHODS:The serum and urine samples were collected from 54 patients with HSPN,54 patients with HSP,and 30 healthy control(all from the department of health care of the First Hospital Affiliated to Guangxi Medical University)。All samples were centrifugated and cryopreservated。The content of MMP-9,TIMP-1 in the serum and urine were tested by Enzyme linked Immunosorbent assay(ELISA) from each groups,and the content of Scr,β2-MG in the serum and the content ofβ2-MG in the urine were tested by automatic biochemical analyzer(just for the group of HSP and HSPN).At the same time,the total Protein In 24 hours from HSP group and HSPN group were tested by biuret Colorimetry.Finally,all data were statistically analyzed.RESULTS:(1)Compared with control group,the content of serum MMP-9,TIMP-1,and the ratio of MMP-9/TIMP-1 in HSP group were remarkbly higher than those of control group(P<0.05),the same as the HSPN group(P<0.01);Compared with the HSP group,the content of serum MMP-9, TIMP-1 and the ratio of MMP-9/TIMP-1 in HSPN group were signify-cantly higher(P<0.05)。(2)There is no significant difference in the content of MMP-9,TIMP-1 and the ratio of MMP-9/TIMP-1 in urine between HSP group and normal control group(P>0.05),while the content of MMP-9,TIMP-1 and the ratio of MMP-9/TIMP-1 in urine with HSPN group were Obviously higher than those of normal control group(P<0.05) and the HSP group。(3)The content of serumβ2-MG and urineβ2-MG in HSPN group were significant higher than those of HSP group(P<0.05).There was no significant difference in content of BUN among two groups,the same as SCr(P>0.05).(4)According to content of 24-hour urine protein,the patients with HSPN were divided into 3 groups:group A,urinary protein<0.15g/24h;group B,0.15~1.0g/24h;groupC,>1.0g/24h.The content of MMP-9 and the ratio of MMP-9/ TIMP-1 in serum and urine in group C were remarkbly higher than group A(P<0.01) and group B(P<0.05);but there was no significant difference between group A and group B(P>0.05).The content of serum TIMP-1 in group C was significantly higher than group A(P<0.01),but no significant difference between group A and group B(P>0.05);there was no significant difference between group A,group B,and group C in the content of urine TIMP-1(P>0.05)。(5)Analysis about correlation between the content of MMP-9,TIMP-1 in serum/urine and 24-hour urine protein,serumβ2-MG,urineβ2-MG,BUN,SCr in HSPN patients:The content of serum MMP-9 was positively correlated with 24-hour urine protein,serumβ2-MG,urineβ2-MG(r=0.564,r=0.676,r=0.743, P<0.05),but was negatively correlated with Scr and BUN(r=0.247,r=0.308, P>0.05);The serumal content of TIMP-1 was negatively correlated with 24-hour urine total protein,serumβ2-MG,urineβ2-MG,SCr and BUN(r=0.767,r= 0.376,r=0.483,r=0.294,r=0.197,P>0.05);The urinary content of MMP-9 was positively correlated with 24-hour urine total protein,serumβ2-MG,urineβ2-MG(r=0.787,r=0.536,r=0.450,P<0.05),but was negatively correlated with SCr and BUN(r=0.159,r=0.122,P>0.05);The urinary content of TIMP-1 was negatively correlated with 24-hour urine total protein,serumβ2-MG,urineβ2-MG,SCr and BUN(r=0.553,r=0.467,r=0.309,r=0.271,r=0.337,P>0.05).CONCLUSION:The content of MMP-9,TIMP-1 and the ratio of MMP-9/TIMP-1 in the serum and urine were remarkedly higher in patients with HSPN,and the content of serum and urine MMP-9 were both closely related to the degree of kidney damage.MMP-9 maybe participate in the occurrence and development of HSPN.
