Objective:To investigate the general demographic characteristics,self-efficacy,self-management ability and the life quality level of patients with chronic hepatitis B.To analysis the influence on self-efficacy,self-management ability,the quality of life and biochemical indicators in patients with chronic hepatitis B based on the health education of health management mode.To provide reference for the hospital to carry out the health education and management in patients with chronic hepatitis B.Methods:The study used the convenience sampling method,155 patients with chronic hepatitis B received antiviral treatment were selected in a tertiary comprehensive hospital of Jilin province from April 2014 to June 2017.The health education based on the model of health management was made in the surveyed patients,and information of the patients before intervention,three months after intervention and six months after intervention was collected by the way of questionnaire investigation.The datas of the research were processed and analyzed by SPSS 17.0 software,using frequency and percentage to describe general demographic characteristics of the patients with chronic hepatitis B?using repeated measurements to compare the self-efficacy and self-management,the change of biochemical index and quality of life of the patients with chronic hepatitis B.Results:(1)The general situation of patients with chronic hepatitis B:the majority of the surveyed people were male,accounting for 70.3%;the number of people at the age beyond 51years old was in a majority,accounted for 34.2%;The Han nationality patients accounted for the most,accounted for 60%,and the proportion of other ethnic minorities' patients was 40%.Before intervention,the Self-efficacy of the surveyed people scores(2.53 ±0.40),at the middle level;the self-management ability scores(107.40± 17.88),at the upper-middle-level;the quality of life scores(153.03 ± 15.33),at the upper-middle-level.(2)The comparison of self-efficacy:There were difference among the scores of self-efficacy before intervention,3 months after intervention and 6 months,and the differences were statistically significant(P<0.05).The increased range of the score of 6 months after intervention was higher than that of 3 months after intervention,which indicated the effect of intervention for 6 months was better than 'that of intervention for 3 months.(3)The comparison of self-management ability:There were difference among the scores of self-management ability before intervention,3 months after intervention and 6 months,and the differences were statistically significant(P<0.05).The increased range of the self-management ability score of 6 months after intervention was higher than that of 3 months after intervention,which indicated the effect of intervention for 6 months was better than that of intervention for 3 months.(4)The comparison of quality of life:There were difference among the scores of self-management ability before intervention,3 months after intervention and 6 months,and the differences were statistically significant(P<0.05).The increased range of the quality of life score of 3 months after intervention was lower than that of 3 months after intervention,which indicated the effect of intervention for 6 months was better than that of intervention for 3 months.(5)The comparison of biochemical indexes:Compared with the ALT value before invention,that of 3 months after intervention and 6 months after intervention decreased significantly,and the difference was statistically significant(P<0.05);The HBV-DNA value decreased slightly,but the difference was not statistically significant(P>0.05).Conclusions:The health education based on health management mode is a feasible,effective intervention,which can effectively improve the self-efficacy,self-management ability,biochemical indices of patients with chronic hepatitis B,and ultimately improve their quality of life.With the extension of the intervention time,the effect of intervention is more significant.The health education based on health management mode is suitable to be popularized in the clinical nursing work.
