Construction Of Training Program For Evaluation Ability Of Nurses' Delirium In Intensive Care Unit

ObjectiveTo understand the current situation of nurses ' delirium evaluation in ICU and the current situation of relevant knowledge training,investigate the related needs of deli--rium evaluation ability training,develop a training program for the evaluation ability of nurses' delirium in ICU,in order to improve the nursing ability of nurses in inten--sive care unit.Method1.Questionnaire survey,using convenient sampling method,in December 2016 selected Zhengzhou a three Class A hospital 120 intensive care unit nurses as the research object,uses the General Data adjustment table,the ICU Nurses Delirium Knowledge Examination paper carries on the investigation,before the formal inves--tigation preliminary experiment to the examination paper reliability validity carries on the verification analysis.Statistical methods include: correlation analysis,relia--bility analysis,descriptive analysis,multifactor variance analysis(F-test),Test level ?=0.05.The statistics of gender and title in general data were described by frequency and composition ratio,and the average and standard deviation were used to describe the measurement data,such as age of study object and grade of examination paper.2.Semi-structural interview,using the objective sampling method,in March ~May 2017,selected Zhengzhou a three Class A hospital intensive care unit nurses and severe medical experts as the subjects interviewed,the researcher and interviewed according to the prepared interview outline,the data collected to the information saturation.3.Expert consultation,in June~September 2017 by the use of written consulta--tion,mail consultation,micro-letter consultation,Questionnaire star Counseling in a combination of methods,select the severe medicine,intensive care,nursing manage--ment and nursing education of 17 experts as the object of consultation,the use of Ex--perts questionnaire,The expert advice form for the training programme on the eva--luation ability of nurses in intensive care Unit 2 rounds of consultation with experts.The positive coefficient,concentration degree,coordination degree and authority de--gree of the expert,and the statistical description of each index by means of mean number,standard deviation,coefficient of variation and so on;the coordination coeffi--cient adopts Kendall test.Results1.Questionnaire survey,test the validity of the questionnaire,reliability test: Total Cronbach? coefficient is 0.823,internal consistency reliability is higher,factor cronbach? coefficient,Delirium basic concept(gn)0.814;clinical manifestations(bx)0.895;subtype manifestation and incidence(yx)0.928;Hazard(wh)0.907;risk factor(wx)0.876;prevention and treatment measures(yf)0.919;Evaluation Tool Application(gj)0.921,all greater than 0.70,the internal consistency of all factors is higher;validity test: All the questions and factors do spearman correlation analysis,showing that the internal consistency coefficient of each factor is greater than the correlation coefficient with other factors,and the correlation coefficient of each test and its factor is greater than the correlation coefficient with other factors,these two points indicate that the construction validity is higher.The general data survey showed that the age of nurses in intensive care unit was 27.78±2.65 years old,the result of delirium knowledge test showed that the influencing factors of total score of delirium knowledge were different professional titles and ICU working years,whether they had participated in delirium education and training;The influencing factors of the concept score of delirium were different titles and years of working in ICU,whether they had participated in the education and training of delirium,the influencing factors of the scores of clinical manifestations of delirium were different titles.The influencing factors of delirium hazard score are different professional titles and ICU working years,whether you have participated in delirium education and training,and whether the risk factors of delirium are different titles,the prevention of delirium and the risk factors of delirium.The influencing factors of the scores of treatment measures were the working years of ICU with different titles and the influencing factors of the application of delirium evaluation tools were the working years of ICU with analysis of the interview,summed up 3 themes: 1.The ability to assess the delirium of nurses in ICU is insufficient;2.Obstacles to the assessment of delirium in intensive care units;3.Training programmes for the evaluation of nurses ' delirium in ICU.3.Expert consultation,the average age of Experts 42.82±3.57(36~52)years old;average working life 21.41±3.74(15~30),bachelor degree of 13 people,Master 4 people,the nature of the work of nursing management 5 people,2 patients with severe medicine,6 people,nursing Education 4 people,expert positive coefficient 100%,The average authority coefficient of experts is 0.79±0.06,the authoritative coefficient in Nursing management field is 0.82±0.06,the authoritative coefficient in the severe medical field is 0.78±0.04,the authority coefficient of the serious nursing field is 0.78±0.08,and the authoritative coefficient of nursing education field is 0.76±0.03;Expert opinion Coordination factor: the first round of the overall 5.1 Literature Retrieval training.5.Scientific research ability training.The training methods,time,hours,objectives and evaluation methods are determined.ConclusionThe influencing factors of the total score of delirium knowledge and the scores of each factor are the working years of ICU and whether or not they have participated in the training of delirium education;There are obstacles in the evaluation of delirium in intensive care unit,and the evaluation of nurses ' delirium in ICU requires perfect training program.The Expert consultation constructs the training project,the content,the goal,the way and the school hours,finally builds the training plan.
