[Objective]To observe the effects of Danshen Chuanxiongqin injection on acute myocardial infarction(AMI)patients after following PCI and the model of myocardial infarction in rats.To explore the mechanism of anti-inflammatory and anti-oxidative stress of Danshen Chuanxiongqin injection in AMI.[Method]1.84 patients with AMI after following PCI were divided into two groups randomly and averagely.Both the control group and the experiment group were given secondary prevention program of AMI while the experiment group was given additional therapy by Danshen Chuanxiongqin injection.2.The clinical efficacy was evaluated according to the scores of Traditional Chinese Medicine(TCM)syndrome,the standard score symptoms and the degree of disease grading score grading standards of disease.3.The changes levels of myocardial enzymes cTnT and CK-MB,BNP,MMP-9,CRP,MDA,SOD and NO were measured at different time points after admission.Operation.Left ventricular end-diastolic(LVEDD)and ejection fraction in both groups were recorded.measured by ultrasound.4.Routine blood tests,liver function and kidney function examination were conducted in both groups.5.Rat model of AMI model was established and the model rats were divided into three groups randomly.The experiment group were treated with Danshen Chuanxiongqin by ligustrazine injection,while the AMI group and the sham operation group were disposed with sodium chloride physiological saline.6.Changes of myocardial EF and FS in rats were observed using ultrasound.7.The blood levels of cTnl,MDA and SOD in 3 groups were measured by Elisa.8.The expressions of MMP-9 and MMP-2 in the infarcted myocardium tissues of 3 groups were detected by Western Blot.[Result]1.The total effective rate was 88.10%in the experiment group with 13 patients getting recovery completely and 15 patients obtaining significant efficacy and while 78.05%in the control group.The efficacy in both groups have significant difference(P<0.05).There was statistical difference in the efficacy(p<0.05).2.Symptoms such as chest pain,chest tightness,upset,manic and numbness were significantly relieved in the experiment group.statistical difference was observed when compared with the control group with(P<0.05).3.Cardiac function test was used to detect the heart function.The heart function of patients Patients with grade I of heart function have increased while the heart function of patients with grade IVof heart function have decreased.The heart function of patients with grade I of heart function in the experimental group was 47.61%while 40.47%in the control group.The heart function of patients with grade IV was 5%while 7.5%in the control group.But no statistical differences were observed in both groups.4.There were no significant differences in the serum levels of CK-MB,cTnT and BNP in both groups compared with in pre-operation state(P>0.05).The serum levels of of CK-MB,TnT and BNP were significantly lower than that of the control group(P<0.05)after 7 days’ treatment.No significant differences were observed in the serum levels of MMP-9 and CRP(P>0.05).The serum levels of MMP-9 and CRP decresed significantly compared with those in pre-operation state of both groups.The LVEDD and ejection fraction in the experimental group were improved significantly compared with the control group(P<0.05).5.There were no significant differences were observed in WBC,RBC,PLT and Hb of preoperative and postoperative in both groups compared with pre-operation state(P>0.05).6.By morphological comparison,we found the Danshen Chuanxiongqin group were more plump in shape and more rosy in color compared with the AMI group,but no less plump or less rosy than the sham operation group.ultrasound showed that EF and FS in the AMI group were significantly lower than those rats in the sham operation group(P<0.05).statistical differences were observed in the EF and FS between the Danshen Chuanxiongqin group and the sham operation group(P<0.05).And significant differences were observed in the EF and FS between the Danshen Chuanxiongqin group and the AMI group(P<0.05).7.No significant differences were observed in the serum levels of cTnI,MDA and SOD between the Danshen Chuanxiongqin group and the AMI group before treatment(P>0.05).After treatment,significantly decrease were observed in the serum levels of cTnl and MDA in the Danshen Chuanxiaoqin group were significantly decrease compared with the AMI group(P<0.05).The serum level SOD in the Danshen Chuanxiaoqin group was significantly higher than rats in the AMI group(P<0.05).8.The expression of MMP-2 and MMP-9 in the sham operation group were significantly lower than rats in the AMI group(P<0.05).The expression of MMP-2 and MMP-9 in Danshen Chuanxiaoqin group was significantly lower than rats in the AMI group(P<0.05)[Conclusion]1.It is suggested from our clinical study that the Danshen Chuanxiongqin injection can improve the symptoms of patients of AMI following PCI and significantly reduce the level of myocardial injury markers such as TnT,CK-MB and reduce the expression of MMP-9 and CRP which reduce the inflammatory damage to myocardial.What is more,the Danshen Chuanxiongqin injection can increase the serum level of SOD,decrease the serum level of MDA,and thus to which enhances the body’s anti-oxidative stress injury ability.2.It is suggested from our animal experiments that the Danshen Chuanxiongqin injection can increase the level of FS,EF of rats and reduce the serum level of cTnl of rats.Secondly,the Danshen Chuanxiongqin injection can increase the body’s anti-oxidative stress injury ability by down-regulating the expression of MDA and up-regulating the expression of SOD.Meanwhile,the Danshen Chuanxiongqin injection can reduce the myocardial inflammatory damage by down-regulating the expression of the inflammatory factors such as MMP-2 and MMP-9.
