The Effect Of Yiqi And Blood Combined With Entecavir On Liver Stiffness Of Cirrhosis With Decompensated Hepatitis B

ObjectiveTo prospectively observe the influence of the therapy "yiqihexue decoction"(supplementing qi and activating blood circulation)combined with entecavir treatment on liver stillness of HBV-induced decompensated cirrhosis and evaluate the effectiveness of the proposed method in treatment of HBV-induced decompensated cirrhosis.MethodsWe enrolled 56 HBV-induced decompensated cirrhosis hospitalized in Beijing Chinese Medicine Hospital from 2016.3 to 2017.8,who were treated with "yiqihexue decoction"combined with 0.5mg/day ETV,and evaluated the clinical outcomes.Using self-contrast method,we observe the change of liver stiffness and clinical symptoms,liver function,HBV-DNA,blood routine examination,prothrombin activity to explore the effective treatment of HBV-induced decompensated cirrhosis by combination of Chinese traditional and western medicine.Results1 Symptom integral:After lmonth,3 monthsand 6months of treatment,the symptom integral decreased by 15%(P<0.01),34%(P<0.01),61%(P<0.01)respectively.The total effective rate was 96%.2 Liver function:After 6 months of treatment,ALT、AST、GGT、ALP、TB decreased significantly(P<0.01),ALB、CHE increased significantly(P<0.01);3 PTA:After lmonth,3 months and 6 months of treatment,PTA increased by 8.09%(P<0.01),14.83%(P<0.01),22.02%(P<0.01)respectively.4 Blood cells:Afterlmonth,3 months and 6 months of treatment,WBCincreased by 3.75%(P>0.05),10.88%(P<0.05),17.97%(P<0.01)respectively.Afterlmonth,3 months and 6 months of treatment,NEincreased by 4.31%(P>0.05),16.82%(P<0.05),20.14%(P<0.01)respectively.After 1 month,3 months and 6 months of treatment,PLTincreased by 8.04%(P>0.05),14.42%(P>0.05),19.23%(P<0.05)respectively.There was no significant change in RBC and HGB(P>0.05).5 HBV-DNA:After 6 months of treatment,the negative rate of HBV-DNA was 100%.6 Liver elasticity:After 1 month and 3 months of treatment,the liver elasticity value decreased by 3.4%(P>0.05),13.62%(P>0.05)respectively.After 6 months of treatment,the liver elasticity value decreased by 23.19%(P<0.05);The higher the initial liver elasticityis,the more elasticity of liver decrease after treatment.7 Size of spleen and width of portal vein:After Imonth and3 months of treatment,the thickness of spleen decreased2.08%(P>0.05),5.25%(P>0.05)respectively.After 6 months of treatment,the thickness of spleendecreased by 11.24%(P<0.01).After 1month and 3 months of treatment,the diameter of spleen decreased 4.11%(P>0.05),5.05%(P>0.05)respectively.After 6 months of treatment,thediameter of spleendecreased by7.18%(P<0.01).After 1month and 3 months of treatment,the width of portal vein decreased 1.65%(P>0.05),3.99%(P>0.05)respectively.After 6 months of treatment,thewidth of portal vein decreased by 6.81%(P<0.01).ConclusionTherapy"yiqihexue decoction"(supplementing qi and activating blood circulation)Combined with Entecavir treatment can significantly improve the clinical symptoms of patientsand improve the synthesis and reserve function of the liver,to improve the counting of WBC,NE and PLT,toeffectively inhibit the replication of HBV,and to decrease the liver elasticity value of HBV-induced decompensated cirrhosis,and reduce the size of the spleen and the width of the portal vein.It is worthy of clinical promotion.
