Objective:To analysis the change of gut microbiota in patients which with hyperglycemia in early pregnancy,and to explore the relationship between gut microbiota and insulin resistance.Methods:In the present investigation,50 pregnant women before their second trimester were recruited during their regular prenatal visits at Maternity and Child Health Center of SuZhou Municipal Hospital between 1~stt November 2015 and 31~stt October 2017.28 pregnant women whose non-abnormality of glucose metabolism was selected as control group.The rest of the subjects had glucose metabolism disorder.We collected the data of age,height,weight and pre-pregnancy weight of each study participant,and detected biochemical variables such as fasting glucose,HbA1c,fasting insulin,C-peptide and other indicators.Meanwhile,we extracted DNA of gut microbiota from the stool samples,and analyzed distribution of gut microbiota by 16s rRNA gene sequencing.Results:The abundance and diversity of the OTUs,Chao,Ace and Shannon of gut microbiota in hyperglycemia patients during early pregnancy were significantly lower than that in pregnant women with normal glucose tolerance.On the contrary,the index of Simpson in hyperglycemia patients was higher than that in healthy.The composition and proportion of intestinal flora in the two groups was obviously different.At the order level,Clostridiales and Desulfovibrionales were more abundance in the control group,but at the level of family,advantage bacterium group was Ruminococcaceae and Prevotellaceae.At the class level,insulin or HOMA-IR had a positive correlation with the abundance of Erysipelotrichia(phylum Deinococcus-Thermus)(R=0.541,P<0.05),hadanegativecorrelationwith Negativicutes(phylum Bacteroidetes)(R=0.714,P<0.05)reversely.There was a positive correlation between HOMA-IR with the abundance of Coriobacteriales(phylum Proteobacteria)and Erysipelotrichales(phylum Firmicutes)(R=0.037/0.046,P<0.05).Conclusions:The component proportion of intestinal flora in the patients with hyperglycemia during early pregnancy was disregulated and the flora abundance and diversity decreased.At the level of order,the advantage bacterium of control group was Clostridiales and Desulfovibrionales while at the family level was Ruminococcaceae and Prevotellaceae.Insulin resistance and pre-pregnancy weight was related to Bacteroidetes,Firmicutes and Proteobacteria.While Bacteroidetes was less abundant or Firmicutes and Proteobacteria were more abundant,the sensitivity to insulin would decrease.
