Objective:The serum levels of CA724,CA50,CA242 and CRP in patients with gastric cancer were determined by the way of combination and their clinical value of combined detection was analyzed in the diagnosis of gastric cancer.Methods:In the present study,105 patients of gastric cancer came from the Affiliated Hospital of Qinghai University from November 2015 to November 2017 were involved.All of them were diagnosed by the pathology of gastroscopy and biopsy,and were servered as an observation group.At the same time we choosed another 147 healthy persons in the physical examination center as control group.The serum levels of CA724,CA50,CA242 of all patients were detected by electrochemiluminescence immunoassay and that of CRP were detected by scattering immunoturbidimetry respectively.Then,the date of the serum levels of these four indexes were collected and analyzed.Last,we evaluated the value of the diagnosis of gastric cancer by using the Receiver Operating Characteristic curve.Results:1.The serum levels of CA724,CA50,CA242 and CRP in the observation group were higher than those in the control group(P<0.05).And those in stage III and IV patients of the observation group were higher than those in patients with not only stage I and II in the observation group but also the control group(P<0.05).The serum levels of CA50,CA242 and CRP in patients with stage I and stage II were significantly higher than those in the control group(P<0.05).2.The serum levels of these four indexes in the patients with lymph node metastasis were significantly higher than those in the patients without lymph node metastasis(P<0.05).3.The predictive sensitivity of the serum CA724,CA50,CA242 and CRP for gastric cancer was 47.62%,40.95%,45.71%,30.48% respectively.The predictive specifity of these four indexes was 78.24%,95.92%,94.56%,95.92% respectively.The accuracy was 65.48%,73.02%,74.21%,68.65% respectively.And the positive likelihood ratio was 2.19,10.04,8.40,7.47 respectively.The combined detection of CA50,CA242,CA724 and CRP has the best sensitivity(73.33%)and the combination of CRP and CA50 has the highest positive likelihood ratio(6.77)when these four indexes were determined in the different ways of combination.4.The analysis of the value of ROC curve showed that the AUC of CA50,CA724,CA242 and CRP was 0.706,0.610,0.827,0.683 singly detected.And the AUC of CA242 was higher than that of CA50,CA724,CRP(P<0.05).The AUC of the combined detection of CA50,CA724,CA242 and CRP in different ways was higher than that of any single index detection for gastric cancer,and the best way of combination was CA242 and CRP(0.867).Conclusion: The abnormal elevation of serum levels of CA724,CA50,CA242 and CRP has a certain significance in the diagnosis of gastric cancer,and the levels of serum CA724,CA50,CA50,CA242 and CRP are positively correlated with the stages of gastric cancer and lymph node metastasis.The combined detection of four index in different ways can improve the sensitivity of any single detection of gastric cancer diagnosis.The diagnostic value of combined detection of CA 242 and CRP is the highest.
