Comparison Of The Effect Of Different Stages Of Pyeloplasty In The Treatment Of Severe Hydronephrosis In Infants

Objective For severe hydronephrosis caused by PUJO in infants under 6months,it is controversial to choose stageⅠamputated pyeloplasty or stageⅡpyeloplasty(after pyelostomy).The clinical effects of stageⅠandⅡstage amputated pyeloplasty were analyzed retrospectively,and provide clinical basis for the rational selection of surgical methods.Methods The data of 69 cases of severe hydronephrosis caused by PUJO from January 2011 to February 2017 in the General Hospital of Ningxia Medical University were collected.According to the procedure,57 cases were treated with stageⅠpyeloplasty(n=57)and 12 cases(n=12)with stageⅡpyeloplasty.Comparative analysis of two groups of data:sex,nationality,age,anterior and posterior diameter of renal pelvis,thickness of renal cortex,renal function,total hospitalization time,total hospitalization cost,operative time,intraoperative bleeding volume,postoperative complications.the clinical effect of two surgical methods on severe hydronephrosis caused by PUJO in infants was analyzed.The data were analyzed by SPSS 20.0 software.The measurement data between the two groups were expressed as mean±standard deviation(χ±S).T test was used to compare the data andχ~2 test was used to compare the count data.Results 1.Gender,age,nationality,anterior and posterior diameter of renal pelvis,thickness of renal cortex before operation,renal function before operation and renal function after operation were not significantly different between stageⅠoperation group and stageⅡgroup(P>0.05).2.StageⅠandⅡgroups:the average operation time was 107.81±9.67 min and 124.17±10.62 min,the mean intraoperative bleeding was 12.16±2.52ml/15.83±2.94 ml,the average total hospitalization time was 15±5 d/19±4 d,the average total hospitalization cost was 12180.18±3124.99 yuan/16157.67±4198.34 yuan,respectively.There was significant difference between the two groups(P<0.05).Conclusion There was no significant difference between the two methods in the recovery of renal function of severe hydronephrosis caused by ureteropelvic junction obstruction in infants under 6 months.Compared with stageⅡoperation,the operation time,total hospitalization time and postoperative infection rate and total hospitalization cost of stageⅠdirect pyeloplasty were shorter and lower.
