Objective To study the clinical value of the cell classification of bronchial alveolar lavage in interstitial lung disease. Methods 100 patients with suspected interstitial lung disease who were admitted from October 2015 to October 2017 in Ningxia Hui Autonomous Region People’s Hospital were selected for bronchoalveolar lavage and bronchoalveolar lavage fluid(BALF)was used for cell sorting and lymphocyte subsets.Group analysis, the final diagnosis by clinical history, imaging findings, immune serum and lung biopsy. Results BAL examination was performed on all patients and BALF was collected for cell sorting: the IPF group neutrophils was significantly up to 16%,which was significantly different from COP,CTD-ILD,and sarcoidosis(P<0.01).Poorly,the CTD-ILD group was slightly elevated and there was no statistically significant difference from the IPF(P>0.05).The lymphocyte counts in sarcoidosis,COP,and CTD-ILD were all elevated;the increase was significant in order: sarcoidosis,COP,and CTD-ILD.There was no statistically significant difference between the groups. The effect of hormone therapy was significant;CEP Eosinophils were significantly elevated by 34% and were effective for hormone therapy.Analysis of BALF lymphocyte subsets in all sarcoidosis and some other groups of ILD patients revealed that the highest CD4/CD8 mean value was 3.45 in patients with sarcoidosis. Conclusions The classification of bronchoalveolar lavage fluid cells has some value in the diagnosis of ILD.Combining lymphocyte subsets has a certain clinical value in the diagnosis of sarcoidosis. At the same time,it has a certain guiding effect on the responsiveness of ILD hormone therapy.
