Objective: To investigate the clinical value of percutaneous transforaminal endoscopic spine system(TESSYS)with traditional small incision discectomy surgery in lumbar discectomy for disc herniation.Methods: Analyzed retrospectively from June 2014 to June 2015 and conforms to the diagnosis of lumbar intervertebral disc protrusion during clinical research into the standard of clinical manifestation and imaging data of 100 patients with surgery,according to the operation method divided into the observation group of percutaneous transforaminal endoscopic spine system(TESSYS)with the control group of small incision lamina windowing traditional surgery treatment,50 cases in each group.Cases observation records in the two groups,respectively,the operation time,intraoperative blood loss,postoperative bed time,postoperative hospitalization days,preoperative and postoperative 24 hours for 24 hours,48 hours,3 months,6 months,12 months follow-up,and USES the digital evaluation questionnaire(numerical rating scale,NRS)and modified Mac Nab criteria evaluation of curative effect,use the statistical methods such as chi-square test,t test,curative effect analysis.Results: Observation group operating time(61.8 ± 6.3 min)compared with control group(59.7 ± 8.4 min)have no obvious difference;Bleeding amount of observation group(8.6± 2.8 ml)(2.5 ± 0.5 h),postoperative bed time,postoperative hospitalization days(3.2 ±0.5 d)significantly less than the control group(32.6 ± 7.9 ml;50.4 ± 6.9 h;12.5 ± 2.8 d)(P < 0.05);Observation group after 24 hours NRS score(1.84 ± 0.92),12 months NRS score(1.32 ± 0.94)was significantly lower than control group(2.78 ± 0.58;1.64 ± 1.80)(P < 0.05),but the observation group before surgery NRS score(7.8 ± 0.76),postoperative NRS score(1.34 ± 0.62)for 48 hours,3 months NRS score(1.29 ± 0.59),and 6 months of NRS score(1.20 ± 0.45)and control group(7.6 ± 0.83;1.42 ± 0.72;1.32± 0.64;1.28 ± 0.62)is no statistical difference(P > 0.05).Observation group after 12 months modified Mac Nab criteria significantly better than that of control group(P <0.05),but after 3 months,6 months modified Mac Nab criteria no statistical difference compared with control group(P > 0.05).Conclusion: From the recent curative effect analysis,the curative effect of the two kinds of surgical methods,mid and curative effect of observation group was better than the control group.Percutaneous intervertebral foramen mirror TESSYS technology has a small surgical trauma,inflammatory reaction,weight,less blood loss,high precision operation,postoperative function recovery,away from bed time is short,and shorter hospitalization time,does not affect the improvement of symptoms and signs,postoperative without oral analgesic drugs,damage to bony structure light even not to undermine,maintain the normal anatomy of the lumbar spine and maintain the advantages of biomechanical stability to speed up the recovery,at the same time in light of soft tissue around the spinal canal,nerve root stimulation light,mid postoperative recurrence rate is low.
