| Background:Ovarian cancer is one of the most common gynecological malignancies in women.The highest mortality is caused by ovarian cancer in gynecological malignant tumor.Epithelial ovarian cancer(EOC)accounted for 85%of the incidence of ovarian malignant tumors.Regarding to the young patients of epithelial ovarian cancer with stage I,whose survival rate could reach to 90%.It is estimated that approximately 12%of new cases of ovarian cancer were diagnosed under 45 years old in the western countries.While in the Europe and US,more than 10,000 new cases of malignant ovarian neoplasms per year were women of childbearing age.In the past decade,approximately 12%women with first pregnancy were over 40 years old,which means that many patients would have suffered from ovarian cancer before their first pregnancy.Recently,with the development of diagnosis and treatment and the improvement of quality of life,more and more young patients with ovarian cancer have a strong willing of preserving function of fertility.Therefore,how to optimize the therapeutic strategies in clinical practice to preserve the function of fertility and to cure the ovarian cancer with a favorable prognosis is a crucial issue to be resolved.Objective:This article will discuss several issues about the fertility preservation surgery in young women with ovarian cancer,including the indications,safety,outcomes of pregnancy,whether retaining the reproductive organs after pregnancy,and prognosis.We hope to bring some good news to those young EOC patients who are eager to preserve their function of fertility,and make it possible for them to retain their reproductive function.Method:We summarized the therapeutic procedures and follow-up results of a young patient with EOC which was admitted in our hospital in 2016.The clinical data of this case were reported in detail and made a literature review according to the latest investigations all over the world.Results:This patient was diagnosed as moderately differentiated mucinous cystadenocarcinoma with stage IA base on the guideline of FIGO,and she expressed a strong desire to preserve the function of fertility.When we fully informed the advantages and disadvantages to the patients and her family,we conducted the fertility preservation surgery based on the surgical indications.Furthermore,the patient was treated with docetaxel plus carboplatin chemotherapy for 6 courses.Finally,the tumor markers and imaging data showed no proof of recurrence and the level of sex hormone returned to normal condition during the 2 years of follow-up,and she is still in close follow-up now.Conclusion:With the continuous development of the concept of value medicine and the deepening of humanized treatment of cancer,those young patients with EOC that eager to retain fertility function and have surgical indications for preserving fertility function can choose for fertility preservation surgery,which does not increase the recurrence rate or decrease the survival time after surgery.Besides,it also improves the quality of life in young patients with EOC who desire to retain fertility. |
