| Objective: The aim of this study is to evaluate the safety and effectiveness of fertility-sparing surgery as the treatment for patients with malignant and borderline ovarian tumor,and to investigate the factors related to the ability of recurrence and pregnancy.Methods: A retrospective study of young malignant and borderline ovarian tumors inpatients(≤40 years old)was performed between January 2005 and January 2016 in The Second Hospital of Hebei Medical University,the survival and recurrence rate,surgery approach and type,postoperative chemotherapy,staging surgery or not,postoperative menstruation and pregnancy outcomes were analyzed.Results: Datas were analyzed from 51 patients treated with fertilitysparing surgery with a mean age was 25.5±5.8(12-39)years.The most frequent type was borderline tumors 23 patients(45.1%)and the second was malignant ovarian germ cell tumors 10 patients(19.6%),then the epithelial ovarian carcinoma(EOC)9 patients(17.6%),finally the Sex cord stromal tumor and Ovarian cancer sarcoma was 8 patients(15.7%)and 1 patient(2%).46 were followed up,the median follow-up time of 15 months(6-137 month).Five patients(10.9%)showed carcinoma recurrence for median 7(1-17)months.2(3.9%)Sex cord stromal tumor patients died of the disease when 9 and 75 months.The increase of the tumor marker had a significant effect on recurrence(P≤0.05).Of 22 patients who want pregnancy,18 pregnancies among 15 patients were achieved by the end of the follow-ups and the median pregnancy time was 5(0-24)months.Conclusion: Fertility-sparing treatment for patients with malignant and borderline ovarian tumor is safe with promising reproductive outcomes.Cautious the patients that the tumor marker high after the surgery.Standard chemotherapy seems have an evident impact on the Sex cord stromal tumor patients,but this still needs for much larger scale studies and longer-term follow-up. |
