Objective:The purpose of this study was to investigate the effect of Anal pain fumigation lotion combined with acupoint application on the adjuvant treatment of III and IV mixed hemorrhoids.Methods:A single-blind randomized study was carried out a total of 165 patients with mixed hemorrhoids(Ⅲ-Ⅳinternal hemorrhoids with external hemorrhoids)in the Southwest Medical University Affiliated Hospital of Traditional Chinese Medicine from July 2015 to July 2016 were prospectively enrolled in the study and equally divided into 4 groups.All patients were treated with external stripping and internal fixation.The control group was treated with potassium permanganate solution and bathed for 7 days from the second day after the operation;The fumigation group was treated with Anal pain fumigation lotion and bathed for 7 days from the second day after the operation;the applicator group was treated with applicator and bathed for 7 days from the second day after the operation;the combination group was treated with Anal fumigation lotion and applicator for 7 days from the second day after the operation.Pain was scored using a visual analog scale(VAS).Anal incontinence was assessed using the"Incontinence Wexner Scoring System".SPSS19.0 was used for statistical analysis.P<0.05 indicated statistically significant difference.Results:There were no difference between sex ratio(x~2=1.826,P=0.609),age(F=1.619,P=0.1871),weight(F=0.477,P=0.699)and height(F=0.470,P=0.704)among 4groups;There were no difference between stasis classification(x~2=0.955,P=0.812),VAS scores for daily defecation pain(F=0.341,P=0.796)before surgery.The total efficiency of the 4 groups is 100%.The postoperative pain score on the third days in the fumigation group(t=2.100,P=0.039),the patch group(t=2.062,P=0.043)and the combination group(t=4.045,P=0.0001)were all lower than those in the control group;compared with the combination group,The fumigation group(t=2.092,P=0.040)and the application group(t=2.370,P=0.020)were higher than the combined group.The anal edema was observed on the fourth postoperative day,compared with the control group,the edema scores in the fumigation group(t=2.067,P=0.042)and combined group(t=4.791,P<0.0001)were lower than those in the control group.The significance of the study showed that there was no significant difference between the patch group and the control group(t=1.644,P=0.104);the edema score of the combined group was lower than the fumigation group(t=2.096,P=0.039).The hemorrhage scores in the first 3 days after operation were lower than those in the control group,and the bleeding scores in the fumigation group(t=2.126,P=0.037)and the combined group(t=4.684,P<0.0001)were lower than those in the control group.There was no significant difference between the group and the control group(t=0.343,P=0.733).The score of the combined group was lower than that of the fumigation group(t=2.137,P=0.036).There was no statistically significant difference in anal stenosis at 1 month after surgery(x~2=0.475,P=0.924),and there was no significant difference in the Wexner score between the four groups(F=3.259,P=0.065).Compared with the control group,the wound healing time was significantly shorter in the fumigation group(t=2.934,P=0.004),patch group(t=2.131,P=0.036),and combination group(t=6.685,P<0.0001);The washing group(t=3.988,P=0.0001)and the patch group(t=5.098,P<0.0001)were significantly higher than the combined group.Hospitalization costs,patient satisfaction,and postoperative recurrence rate were not statistically significant among the 4groups.Conclusion:Anal pain fumigation lotion has good curative effect on relieving postoperative pain,postoperative edema,postoperative hemorrhage and reducing wound healing time.Application has good curative effect on reducing postoperative pain and reducing wound healing time.Combination therapy is more effective than treatment alone. |
