| Objective:When there is severe pain after milligan-morgan hemorrhoidectomy,give different intensity of electroacupuncture stimulation,observing and recording the clinical efficacy,to select the most appropriate stimulation intensity,In order to further optimize the clinical treatment.Methods:According to the inclusion and exclusion criteria,90 patients with pain after milligan-morgan hemorrhoidectomy were randomly divided into 3 groups,30 cases in each group.On the basis of routine treatment,3 groups patient were given electroacupuncture for analgesia.The intensity of electroacupuncture stimulation was set as electroacupuncture group I(1m A),electroacupuncture group II(3m A)and electroacupuncture group III(5m A)from low to high,respectively.The first treatment was given when the VAS score was ≥4 on the operative day,and the treatment was performed once a day before dressing change on the first postoperative day,for 3 consecutive days,time of treatment lasts 4 days in total.Observing and recording the VAS pain intensity score and limb activity score of the 3 groups of patients On the day after operation and before treatment,1 hour after the treatment after the operation on the day,day 1,2,3 and 7 after the dressing change.After 7 days,the total dose of the analgesic paracetamol and tramadol was taken orally(take the drug when the VAS pain score is>6),The difference of curative effect among the three groups were analyzed.Results:1.VAS score: There was no statistical difference in VAS score among the three groups before and after the treatment on the day after operation(P > 0.05);The VAS score of group II on day 1,2,3 and 7 after operation were all lower than those of group I and group III,When compared with group I,there exsists statistical difference(P<0.05);When compared with group III,the VAS score on day 2 after operation exsists statistical difference from those of group III(P< 0.05),while there was no statistical difference between group II and group III on day 1,3 and 7 after operation(P > 0.05).2.Limb activity score: There was no statistical difference in the limb activity scores of the three groups the day before treatment and after the treatment(P>0.05);the limb activity score of the patients in group II were lower than those in group I on the 1st,2nd and 3rd day after operation.When compared with group I,there exsists statistical difference(P<0.05),When compared with group III,there was no statistical difference on the first and second day after operation(P>0.05),and there was a statistical difference on the third day after operation(P<0.05);the limb activity score was no statistical difference among the three groups on the 7th day after operation(P>0.05).3.The dose of oral paracetamol and tramadol tablets for 7 days after operation: the patients in group II and group III were less than those in group I(P<0.05),and there was no statistical difference between group II and group III(P>0.05).4.Safety evaluation:During the experimental study,one patient in group III of electroacupuncture developed Local siltation marks appear,which he could not tolerate consciously.Therefore,he withdrew from the experiment and fell off.It may be caused by the injury of local capillaries due to the high intensity of acupuncture.There were no adverse reactions such as dizziness and allergy above three groups.Two patients in group I had nausea and other symptoms.They were considered to be related to the side effects of paracetamol and tramadol tablets and were not given special treatment.Conclusion:1.Different intensity electroacupuncture can relieve the pain after milligan-morgan hemorrhoidectomy.3m A and 5m A strength is better,which is better than that of 1m A strength electroacupuncture.with 3m A or 5m A,The analgesic effect of electroacupuncture is roughly the same,when viewing the general trend,3m A electroacupuncture has a better analgesic effect and safer. |
