Objectives:1.To investigate the status of self-perceived burden, diabetes management self-efficacy and quality of life in patients with type 2 diabetes mellitus.2.To analyze the influencing factors of self-perceived burden and quality of life in patients with type 2 diabetes mellitus.3.To explore the relationship between self-perceived burden and quality of life in patients with type 2 diabetes mellitus.4.To investigate the mediating effect between self-perceived burden and quality of life in patients with type 2 diabetes mellitus.5.According to the references and the results of the investigation,to provide reference frame for exploring the systematic nursing measures for the patients with type 2 diabetes mellitus. Methods:A total of 300 patients with type 2 diabetes mellitus in four tertiary hospitals of Xuzhou City were investigated using Self-Perceived Burden Scale(SPBS),Chinese version of Diabetes Management Self-efficacy Scale(C-DMSES)and Diabetes Specific Quality of Life Scale(DSQL)by the convenience sampling method.Results:1.The total score of the self-perceived burden scale was(31.61 ± 8.42).The scores of each dimension from high to low were economic factors, emotional factors and physical factors.2.2.2 the total score of self-efficacy in diabetic management was 135.47 ± 36.55).The scores from high to low were medication, blood glucose monitoring, exercise and diet.3.Univariate analysis showed that, there was a significant difference between the age, marital status, living area, educational level, work status, health status of the main caregivers, family income, medical expenses, number of hospitalizations, duration of illness, presence or absence of complications, whether they received diabetes education on the total self-perceived burden(P<0.05).Multivariate stepwise regression analysis showed that the monthly economic income, medical expenses payment method, the duration of illness, the presence or absence of complications, diabetes management self-efficacy were the influencing factors of self-perceived burden of type 2 diabetes mellitus.4.The total score of the quality of life in patients with type 2 diabetes mellitus scale was(55.27 ± 11.02).The scores of each dimension from high to low were psychological/mental,physiological function, treatment and social relationship.5.The score difference of the quality of life in patients caused by the difference of age, marital status, living area, educational level, work status, health status of primary caregivers, family monthly economic income, payment of medical expenses, number of hospitalizations, the duration of illness, complications, whether they received diabetes education was statistically significant(P<0.05).6.By using Pearson correlation analysis, the results showed that the relationship between self-perceived burden score and quality of life score was negatively correlated. The correlation coefficient r =-0.251,P<0.05.7.Self-efficacy of diabetes mellitus management played an intermediary role in the influence of Self-perceived burden on quality of Life. The intermediate effect was 30.23%Conclusions:1.The self-perceived burden of patients with type 2 diabetes mellitus is generally mild to moderate level.Self-efficacy of Type 2 Diabetes is at medium level. The quality of life of patients is at a low level.2.The main influencing factors of self-perceived burden of type 2 diabetes mellitus include family monthly income, medical expenses payment method, duration of disease, the presence of complications,self-efficacy.3.The score difference of the quality of life in patients caused by the difference of age, marital status, living area, educational level, work status, health status of primary caregivers, family monthly economic income, payment of medical expenses, number of hospitalizations, the duration of illness, complications, whether they received diabetes education was statistically significant.4.The relationship between the self-perceived burden and the quality of life in patients with type 2 diabetes mellitus is negatively correlated. In patients with type 2 diabetes mellitus.5.Self-efficacy is an important intermediary variable between self-perceived burden and quality of life.
