Clinical Study On Treatment Of Acute On Chronic Liver Failure(Syndrome Of Yang Deficiency And Stasis Toxicity) By Modified Yinchen Sini Decoction

Objective: To observe the clinical efficacy of Modified Yinchen Sini Decoction in the treatment of acute on chronic liver failure(ACLF)(syndrome of yang deficiency and stasis toxicity),in order to provide the evidence for the clinical treatment of that with traditional Chinese medicine.Methods: 60 patients with ACLF(syndrome of yang deficiency and stasis toxicity)were enrolled in the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine(TCM)from March 2016 to November 2017.The random number was generated by computer.All the patients were randomly divided into treatment group and control group,with 30 cases each group.The control group was given the basic treatment of western medicine.The MYS group was treated with Modified Yinchen Sini Decoction based on the control group.The two groups were treated for 8 weeks.Before and after the treatment,the mortality,liver function,coagulation function,Chinese medicine syndrome score and MELD score were observed,the clinical efficacy of was observed as well.Results:(1)After 8 weeks,Modified Yinchen Sini Decoction combined with western medicine treatment(MYS)group can reduce the mortality rate of ACLF,the curative effect is better than the combination of western medicine treatment(control)group(P < 0.05).The total effective rate of the MYS group and the control group were 66.67% and 43.33%,respectively and the effective rate of the MYS group was better than that of the control group.The difference between the two groups was statistically significant(P < 0.05).(2)After 4 and 8 weeks of treatment,the levels of TBIL in the MYS group and the control group were statistically significant(P = 0.003,P = 0.029).The TBIL value of the MYS group was better than that of the control group.(3)After 4 weeks of treatment,there was significant difference in PTA levels between the two groups(P < 0.05).The improvement of coagulation function in the MYS group was better than that in the control group.There was no significant difference in the PTA levels between the two groups(P > 0.05).(4)After 4 and 8 weeks of treatment,the scores of TCM syndrome in treatment group and control group were significantly decreased,there was significant difference between the two groups(P < 0.05).The TCM symptom score in the MYS group was better than the control group.(5)After 4 weeks and 8 weeks of treatment,the MELD scores decreased in both groups(P < 0.05),MELD score improved in the treatment group better than that of in the control group.Conclusions: MYS can improve the clinical symptoms of ACLF(syndrome of yang deficiency and stasis toxicity),protect the liver,promote the recovery of blood coagulation mechanism,improve the short-term prognosis,reduce the mortality in 8 weeks.The overall clinical efficacy is better than a single western medicine treatment.
