Objective: To explore real-time three-dimensional echocardiography measured normal children the feasibility and accuracy of left ventricular systolic function and clinical application in pediatric cardiac function evaluation value.Methods: We collected 305 cases of children who underwent routine echocardiography during the period of 2015.6-2016.10 in our hospital.All the patients were measured using two-dimensional ultrasound,three-dimensional echocardiography and real-time arbitrary multi-plane technology for left ventricular systolic function.Difference and correlation between the measured results and two-dimensional Simpson with the real-time arbitrary multi-plane technology method on the operating time by t test and the difference of the bivariate Pearson method is analyzed.According to the surface area and age,children were grouped using the single factor analysis of variance between the full volume three-dimensional data between different groups,and stepwise multiple linear regression analysis of gender,age,body surface area of real-time left ventricular systolic function by full volume three-dimensional parameter index.The consistency of the results of real time three-dimensional ultrasound measurements between different observers and the same observers was analyzed using Bland-Altman method.Result:(1)There were correlation between in EDV,ESV,SV and EF of left ventricular systolic function parameters measured by two dimensional Simpson method and real time three dimensional full volume method(r=0.967,0.955,0.954,P<0.05).Real time three dimensional full volume measurement of EDV,ESV,SV value is greater than two dimensional Simpson method,EF value is less than two dimensional Simpson method,the difference was statistically significant(P < 0.05).(2)There were correlation between in EDV,ESV,SV and EF of left ventricular systolic function parameters measured by Two dimensional Simpson method and real-time arbitrary multi-plane Simpson(correlation coefficient r = 0.976,0.969,0.966,0.969,P < 0.05);Measured both EDV,ESV,SV,EF value no statistical difference(P > 0.05).Two-dimensional Simpson method operating time more than real-time arbitrary multi-plane Simpson method,the difference was statistically significant(P < 0.05).(3)There was no significant difference in gender of EDV,ESV,SV(P> 0.05).There were significant difference between different age groups(P <0.05).With the increase of age,the values of EDV,ESV and SV gradually increased,and there were no significant differences in EF between all age groups(P >0.05).There were significant differences between different body surface area groups(P <0.05),With the increase of body surface area,the values of EDV,ESV and SV were gradually increased,There were no significant differences in EF between different body surface areas(P> 0.05).(4)There were positive correlation between the age with the three dimensional left ventricular systolic function parameters EDV,ESV,SV(r=0.89,0.885,0.885,P<0.05),There was no correlation between age and the EF values(P >0.05),There was no correlation between EDV,ESV,SV and EF between gender and three-dimensional ultrasonography(P> 0.05).There was a positive correlation between surface area and EDV,ESV and SV(correlation coefficient r = 0.956,0.95,0.95,P <0.05),And there was no correlation with EF value(P >0.05).Compared with age and sex,body surface area had the greatest effect on parameters(P <0.05).(5)There were no significant differences in the parameters of left ventricular systolic function measured by real-time three-dimensional volume method between the same surveyor and the different subjects(P <0.05),the correlation between the data group and the correlation group were more than 0.9,But the difference between LVEF between the same surveyor and the different surveyor is large.Conclusion:(1)Real-time three-dimensional echocardiography is an accurate and feasible method for measuring left ventricular systolic function in normal children.(2)Three-dimensional left ventricular systolic function parameters of children with children’s growth showed a certain regular changes: EF value did not change with age,gender and body surface area,EDV,ESV and SV increased with age and body surface area.(3)Real-time arbitrary multi-plane technology to a certain extent improve the efficiency of ultrasound physicians,can be widely used in pediatric clinical.
